Product Information
The product is a model #5016OSCLMWP light fixture from Crystorama. It has a damp rating and is designed to be used with 6 candelabra base bulbs. Product Usage Instructions
- Before installation, place the fixture parts on a soft cloth-covered platform.
- Turn off the power before starting the installation.
- Ensure that all electrical components are installed by a licensed electrician in accordance with the National Electric Code and local electrical codes.
- Install the mounting bar onto the outlet box.
- Spread the arms out equally.
- Insert the recommended candelabra base bulbs (6 in total) into the fixture. Ensure that the wattage does not exceed 60 watts per bulb.
- If using LED bulbs, make sure they are compatible with the fixture and follow the manufacturer’s instructions for extended use.
- Attach the crystal sets to the designated locations according to the provided quantities.
- Refer to the part numbers listed for each component and follow the assembly instructions for proper installation.

You will need 6- Candelabra Base Bulb 40 Watts recommended 60 Watts Max. USE LED BULBS FOR EXTENDED USE The fixture is Damp rated
HOW TO INSTALL
Place the fixture parts on a soft cloth-covered platform before installation of fixture.
- Install the mounting bar onto the outlet box.
- Spread the arms out equally. Place the distributor box cap over the top of arms. Make sure the slot fits over the arm.
- Place the cover over the distributor box kit and secure with fixture loop.
- Decide the desired hanging height and adjust the chain to the correct length.
- Insert the connector in between the bottom of arm. Secure it with ball nuts.
- Raise the fixture up and do the wire connection. Attach the canopy over the mounting bar and secure with thread and nut.
- Place the candle sleeve and bobeche over the socket.
- Dress the crystals as illustrated.
Spread the folded arms.
