WARNING

Installation, service, and maintenance of luminaries should be performed by a qualified licensed electrician.
Check the lamp for damage before installation. If the lamp is damaged, do not install it.
If the glass lamp tube is broken, dispose of the lamp immediately.

CAUTION

This LED T5 lamp if for use only with products described and installed in accordance with product specifications. This LED T5 lamp must be operated with a fluorescent electronic ballast.
Only use in place of fluorescent lamps specified on label.
Do not install this lamp in a pre-heat luminaire.
Not suitable for enclosed fixtures.
Not for emergency lighting.
This lamp only operates on electronic ballasts. If the lamp does not light when the luminaire is powered on, remove the lamp from the luminaire and contact the manufacturer or a qualified electrician.
If the lamp or luminaire exhibits undesirable operation (buzzing, flickering, ect…), immediately turn off the power, remove the lamp from the luminaire, and contact the manufacturer.
Do not place any objects on the lamp.
Keep the lamp away from open flames and high temperatures.

If the glass tube is broken, dispose of the lamp immediately.

NOTICE

Make sure the power of OFF.
Remove the lens or diffuses from the fixture (if appliable).
Remove the existing fluorescent lamps from the luminaire.
Place the LED lamp in the luminaire and insert lamp pins into the luminaire lamp holders.
Rotate the LED lamp until it locks into place. Ensure that the front of the LED lamp points away from the back of the luminaire.

NOTE: Install one LED T5 replacement lamps for each fluorescent lamp removed.
