Product Usage Instructions

Items Included:

The package includes the 4-in-1 Receptacle, mounting screws, grounding conductor, and ground screw.

Wiring and Installation:

WARNING: To avoid fire, shock, or death, turn off power at the circuit breaker or fuse before wiring. Test that power is off before proceeding.

WARNING: Never use with an uninsulated ground conductor to prevent electrical shorts.

Wire Preparation:

  1. Remove the cord jacket, ensuring not to damage the insulation.
  2. Remove insulation on each conductor to expose bare copper wire at each end.
  3. Twist all wire strands together and connect as instructed.

Installation Instructions:

  1. Remove two break-off tabs for split-circuit wiring.
  2. A and B circuit identification is marked on the receptacle face and back cover.
  3. A grounding conductor may be provided to ground the receptacle to a metallic box.

INTRODUCTION

The Leviton® 4-in-1™ Receptacle design saves on materials and on installation time.
For example: you can easily replace a duplex receptacle without the use of an adapter plate. This convenient setup doubles the number of outlets available, while also saving wiring time and money.
The 4-in-1 Receptacle is ideal for renovation and retrofit applications in commercial, institutional and industrial settings. This includes workstations, temporary power needs, computer stations, and hospital and health care areas.
The Isolated Ground (IG) devices help protect sensitive electronic equipment from some problems caused by electrical “noise.”

The Leviton 4-in-1 Receptacle is available in the following configurations:

  • Industrial Specification Grade, UL/CSA
    Ratings: 15A – 125V, UL/CSA; 20A – 125V, UL

  • Industrial Specification Grade, Hospital Grade
    Ratings: 15A – 125V, UL/CSA; 20A – 125V, UL

  • Industrial Specification Grade, Hospital Grade-Isolated Ground
    Ratings: 15A – 125V, UL/CSA; 20A – 125V, UL

The Leviton 4-in-1 Receptacle is UL-listed to mount on a standard single wallbox without an adapter plate. Most comparable receptacles require an adapter plate.

The 4-in-1 Receptacle also fits:

  • Metallic and plastic boxes
  • Octagon concrete ring
  • 4 inch octagon box
  • 4 inch square, surface flush-mounted
  • 4 inch square with plaster ring.

The Leviton 4-in-1 receptacle accepts four (4) Federal Specification-size plugs and/or four (4) Federal Specification angle plugs simultaneously in all outlets. Besides its rugged construction, it has the following features and benefits:

  • Face is marked to show which pair of outlets is being used if two circuits are being fed to the device.
  • Snub hole grip for fast, uniform and reliable wire connection.
  • Backwire ground.
  • Heavy duty T-type/triple wipe contacts.
  • Easy to remove knockouts for quick mounting.
  • Isolated ground pigtail (on IG models).
  • Impact-resistant nylon construction.

ITEMS INCLUDED WITH EACH PACKAGE

4-in-1™ Receptacle:
Cat. Nos. 1254 or 21254; Two #8-32 x .75” mounting screws (head color matches body); Four #8-32 x 1” screws (head color matches body); Two #6-32 x 1-5/8” screws (head color matches body); One 8” solid 12 AWG copper insulated grounding conductor (may or may not be provided); One ground (green hex) screw.

WIRING INSTRUCTIONS

  • WARNING: TO AVOID FIRE, SHOCK, OR DEATH; TURN OFF POWER AT CIRCUIT BREAKER OR FUSE AND TEST THAT POWER IS OFF BEFORE WIRING!
  • WARNING: NEVER USE WITH AN UNINSULATED GROUND CONDUCTOR, AN

ELECTRICAL SHORT MAY OCCUR.
Binding head terminal screws accept up to #12 AWG solid or stranded wire.
For 15-Amp receptacle, use #14 AWG wire.
For 20-Amp Receptacle, use #12 AWG wire ONLY.

To Prepare Wires:
The following wiring instructions apply to all possible mounting configurations. Two break-off tabs MUST BE REMOVED for split-circuit wiring. A and B circuit identification is molded in the receptacle face and on the back cover. A grounding conductor may be provided to ground the receptacle to a metallic box without an existing grounding connection.

  1. Remove the cord jacket about 8”. Be careful not to damage the insulation; the danger of shock or fire exists if the insulation is damaged.

  2. Remove the insulation on each conductor to expose 1” bare copper wire at each end of circuit wires. Keep wire ends straight.

  3. Connect wires as follows: Twist all strands tightly together. Insert the conductor in the snub hole and wrap the conductor counter-clockwise around the terminal screw. For larger size conductors, form a loop with the wire, and place it around the terminal screw. BLACK conductor to BRASS screw, WHITE conductor to SILVER screw, and GREEN conductor to GREEN hex screw. Be sure there are no loose strands. If system ground is uninsulated, then pigtail the insulated ground wire (which may be provided) from the GREEN hex screw to the electrical box.

  4. Tighten terminal screws clockwise to 10 to 12 in-lb.

  5. Mount receptacle (refer to Mounting Instructions for your application).

  6. Restore power at circuit breaker or fuse. Installation is complete

MOUNTING INSTRUCTIONS

Remove knockouts from the 4-in-1 Receptacle, as shown in the following diagrams.
