BEFORE USING THE DEVICE, PLEASE READ THE FOLLOWING INSTRUCTIONS COMPLETELY AND CAREFULLY. CORRECT APPLICATION IS VITAL TO THE PROPER FUNCTIONING OF THE DEVICE.
INTENDED USER PROFILE:
The intended user should be a licensed medical professional, the patient or
the patient’s caregiver. The user or caregiver should be able to read,
understand and be physically capable of performing all the directions,
warnings and cautions provided in the information for use.
INTENDED USE/INDICATIONS
The Procare ComfortForm Back is designed to provide comfort and support of the
lumbar spine. It may be suitable for the symptomatic relief of acute and
chronic low back pain.
PERFORMANCE CHARACTERISTICS
Soft-good/semi-rigid designed to restrict motion through elastic or semi-rigid
construction.
CONTRAINDICATIONS
If you have skin disorders/injuries in the relevant part of the body, the
product should be fitted and worn only after consultation with a medical
professional. Do not use if you are allergic to any of the materials contained
within this product.
WARNINGS AND PRECAUTIONS
- For maximum efficacy, ensure the correct size of strap is selected and the side pulls are adjusted to the required fit.
- If you have experienced any pain, swelling, changes in sensation or other unusual reactions occur while using this product, you should contact your medical professional immediately.
- Do not use over open wounds.
- Do not use this device if it was damaged and/or package has been opened.
NOTE: Contact manufacturer and competent authority in case of a serious incident arising due to usage of this device.
APPLICATION INSTRUCTIONS

Center back pad in the middle of the lower back. Note: The narrow side of the back pad is the top of the support. (Fig.1)

Pull the waistband straps forward and secure in front. Secure the two side elastic support straps on the belt ensuring the lumbar back pad is positioned firmly against the lower back. (Fig.2)

For additional support, pull and secure support strap from left side first, and adhere to the abdominal section of belt. Next, pull the right side support strap, overlapping left side support strap and secure on contact closure. For less support loosen side support straps. (Fig.3)
Optional shoulder straps may be used with support (79-89350). Attach straps to support with suspender buckles. Adjust suspenders for comfort and fit.

CARE
Hand wash in cold water using mild soap, rinse thoroughly. AIR DRY. Note: If
not rinsed thoroughly, residual soap may cause irritation and deteriorate
material.
NOT MADE WITH NATURAL RUBBER LATEX.
INTENDED FOR USE ON A SINGLE PATIENT
WARRANTY DJO, LLC will repair or replace all or part of the unit and its accessories for material or workmanship defects for a period of six months from the date of sale.
