Intended Use
This product is intended to be used as a chair / table to provide positioning
and support of patients during general examinations and procedures conducted
by qualified medical professionals.
Electrical requirements
Caution
When using high frequency surgical devices or endocardial catheters:
- Use non-conductive material to insulate patient from metal portions of the table.
- Consult operating instructions for the device before using in conjunction with drawer or upholstery heater.
Failure to comply may result in electric shock or burns to patient.
Electromagnetic Interference
This Midmark table is designed and built to minimize electromagnetic
interference with other devices.
However, if interference is noticed between another device and this table:
- Remove interfering device from room
- Plug table into an isolated circuit
- Increase separation between table and interfering device
- Contact Midmark if interference persists
Disposal of Equipment
At the end of product life, the table, accessories, and other consumable goods
may become contaminated from normal use. Consult local codes and ordinances
for proper disposal of equipment, accessories and other consumable goods.

Safety Instructions
Caution
Be sure that all personnel and equipment are clear of the table before
activating any function. Failure to do so could result in personal injury.
Hand / Foot Control Connection
There are two hand / foot control cord inlets. One is located on the base
column at the foot end of the table, and one is located on the PC board cover
at the head end. These cord inlets are interchangeable.
To connect hand / foot control cord…
A) Slide narrow portion of cord connector into bracket.
B) Connect cord to inlet (see Note).
C) Tighten locking ring.
Note: The cord connector is “keyed” and must be oriented properly to
connect.
Equipment Alert
Never plug hand/foot control into any other outlets. Doing so may damage
equipment.
Crash Avoidance System
The Crash Avoidance System prevents the table from colliding with the floor.
If the system detects that the headrest mechanism is approximately two inches
from the floor, it will sound several audible “beeps” and disable the
following functions: Base Down, Back Down, and Tilt Up.
Operation
Power To The Table
If the table has no power…
All functions will be inoperable.
Before calling for service…
A) Check power cord connections.
B) Check / reset facility circuit breaker.
C) Check / replace table fuse(s).
Before replacing fuses…
Disconnect power cord from table.
To replace fuses…
[Refer to Specifications section for correct fuse size & ratings] A) Open the
fuse holder cover.
B) Remove fuse holder & faulty fuse(s).
C) Install new fuse(s), then replace fuse holder.
Attention: Voltage setting that shows thru window of fuse
holder cover must match facility supply voltage.
Equipment Alert
Incorrect voltage setting will result in damage to the table.
Base Up / Down
To move the Base UP or DOWN…
Press and hold the appropriate button.
Movement stops when…
Up / Down button is released.
Maximum / minimum position is reached.
Stop button is pressed.
Back Up / Down
To move the Back section UP or DOWN…
Press and hold the appropriate button.
Movement stops when…
Maximum / minimum position is reached.
Up / Down button is released.
Stop button is pressed.
Foot Up / Down
Attention!
The Foot Up function is disabled when the treatment pan is pulled out.
(You will hear several “beeps” when Foot Up is activated with the pan pulled
out.)
To resume Foot Up operation:
Push the pan all the way in.
Press Foot Up button.
To move the Foot section UP or DOWN…
Press and hold the appropriate button.
Movement stops when…
Maximum / minimum position is reached.
Up / Down button is released.
Stop button is pressed.
To Tilt the seat section UP or DOWN…
Press and hold the appropriate button.
Movement stops when…
Maximum / minimum position is reached.
Up / Down button is released.
Stop button is pressed.
Activating Multiple Functions
The BASE / BACK / TILT / FOOT functions may be activated at the same time.
Example:
Base Up / Back Down
To activate multiple functions …
Simultaneously press & hold the desired buttons.
(ex. Base Up & Back Down)
Home Function
The Home function automatically lowers the table base so that your patient may
get on or off of the table.
To lower the base to 23 in. (58.4 cm)…
Press, then immediately release the Home button.
To lower the base to 19 in. (48.2 cm)…
Press and hold the Home button until you hear a single “beep”.
Quick Chair® Function
(w/ Foot Control only)
The Quick Chair® function activates the Base Down, Tilt Down, Foot Down, &
Back Up functions at the same time.
To activate the Quick Chair® function….
Press and hold the Quick Chair® button.
Programming Table Positions
(Programmable Models only)
The Programming feature allows you to easily recall frequently used table
positions.
To program a table position…
A) Move the table to desired position.
B) Press the Program button. (you will hear a single “beep”)
C) Press the desired Position button (1, 2, 3, or 4). (three “beeps”)
Tip: You must press the desired Position button within 5 seconds of
pressing the Program button.
To recall a programmed position…
Press and hold the desired Position button (1, 2, 3, or 4)*.
Tip: The Position buttons (1, 2, 3, 4) can be changed to function as
“press & release”. Refer to: One-Touch Positioning for procedure to
enable. Movement stops when…
Programmed position is reached.
Position button is released.
Stop button is pressed.
One-Touch Positioning
(Programmable Models only)
From serial number V421340 and up
During normal operating mode, the Position buttons must be pressed & held
until the programmed position is reached. When the One-Touch Positioning
feature is activated, the Position buttons function as “press & release”.
To enable (or disable) the One-Touch Positioning feature…
Press & hold the Stop & Program buttons simultaneously.
(after five seconds, you will hear three “beeps” if successful)
To recall a programmed position…
Press the desired Position button (1, 2, 3, 4) for two seconds.
Tip: After you hear a single “beep”, you may release the button & the
chair will continue to move to the programmed position. Movement stops when…
Programmed position is reached.
Stop button is pressed.

Paper Roll
Paper Roll Size (max.)……………………………… 18 in. long x 3.5 in. diameter (45.7 cm x

To install the paper roll…
A) Pivot paper roll cover back to open.
B) Install paper roll, then pull paper over table.
C) Pivot paper roll cover forward to close.
Stirrups
Caution
Never use stirrups to lift table.
Be sure the stirrups are locked in place before using.
The stirrups will not support the patient’s entire weight / excessive force.
To position the stirrups…
A) Pull the stirrup out, then unfold.
B) Lift the stirrup slightly, then move it left or right as desired.
C) Release stirrup to lock in position. Treatment
Pan
Attention!
The Foot Up function is disabled when the treatment pan is pulled out.
(You will hear several “beeps” if Foot Up is activated with the pan pulled
out).
To resume Foot Up operation:
Push the pan all the way in.
Press Foot Up button.
To access the treatment pan…
A) Lower the foot section.
B) Pull the treatment pan slide out. Duplex
Receptacle (optional – not available on 230 volt models)
The duplex receptacle provides power for accessories used during medical
procedures.
Power to the receptacle will be interrupted if the maximum load is exceeded.
Maximum Load [both receptacles combined]………………115 VAC, 3 amps, 50/60 Hz
Equipment Alert
Exceeding the maximum load for the receptacle may result in blown fuses.
If this occurs, all table functions will be disabled until fuses are replaced.

Headrest Positioning
Caution
Be sure headrest is locked securely in position before use.
To adjust the headrest at pivot points A and/or B…
A) Pull upper handle outward to release.
B) Move pivot point(s) to desired position.
C) Push upper handle inward to lock in place.
[For clarity, the upholstery is not shown] Note
The handle tension is adjustable.
Contact your service provider if an adjustment is required.
To adjust the headrest at pivot point C…
A) Pull lower handle outward to release.
B) Move pivot point to desired position.
C) Push lower handle inward to lock in place.
Upholstery Heater
(optional)
The upholstery heater warms the seat and back sections of the upholstered top.
To turn upholstery heater ON / OFF…
Press and release the Upholstery Heater Button on the hand control.
Note: The upholstery heater will…
- reach operating temperature in approx. 15 – 20 minutes.
- automatically shut off after 8 hours.
Equipment Alert
If the back or seat section of the heated upholstery is punctured, it must be
repaired or replaced to ensure proper operation.
Rotational Base
(optional)
To rotate the table base…
A) Press and release the brake pedal.
B) Rotate the table to desired position.
To lock in position…
A) Press and release the brake pedal.
Note: The base will automatically lock in position after two minutes.
Maintenance
Cleaning Upholstery
Equipment Alert
The upholstery is resistant to most medicinal-type stains, but may be damaged
by solvents and dyes. Immediately remove any fluids spilled on the upholstery.
Wash your upholstery weekly with a mild liquid soap and water mixture, rinse
with clear water and dry completely to remove disinfection cleaner build-up.
Disinfect your upholstery with a solution of standard bleach and water mixed 1
in 10 (10%) or chlorine based cleaners. Follow this with a clear water rinse
and thorough drying of material. See current CDC Guideline for Disinfection &
Sterilization in Healthcare Facilities.
To minimize damage caused by disinfectant cleaner residue build-up, do not
allow disinfectants to pool on the upholstery surface. Once the approved
contact time has been obtained, remove and dry excess liquid remaining on the
surface.
Detailed care and maintain instructions are available in your user guide
located on the CD that came with your table or by referring to Upholstery Care
and Maintenance.
Painted Metal / Plastic Surfaces
Clean the painted metal and plastic surfaces weekly using a clean soft cloth,
and mild cleaner. Periodic applications of common furniture wax will ease
cleaning, and maintain the finished luster of the surfaces.
Preventative Maintenance
Periodically inspect the following areas:
- Power cord should be free of cuts or other visible damage.
- All fasteners should be present and tightened securely.
- All mechanical functions should operate properly.
Have a qualified service technician inspect your table every six months.

Warranty Information
SCOPE OF WARRANTY
Midmark Corporation (“Midmark”) warrants to the original retail purchaser that
it will repair or replace components of the domestic and international medical
products manufactured by Midmark (except for components not warranted under
“Exclusions”) that are defective in material or workmanship under normal use
and service. Midmark’s obligation under this warranty is limited to the repair
or replacement, at Midmark’s option, of the applicable components. This
limited warranty shall only apply to defects that are reported to Midmark
within the applicable warranty period and which, upon examination by Midmark,
prove to be defective. This warranty extends only to the fi rst retail
purchas- er of a product and is not transferable or assignable.
APPLICABLE WARRANTY PERIOD
The applicable warranty period, measured from the date of delivery to the
original user, shall be one (1) year for all warranted products and
components.
OBTAINING WARRANTY SERVICE
Warranty service must be obtained through either Midmark or an authorized
dealer in the Midmark product line for which warranty service is requested.

It is the retail purchaser’s obligation to arrange for delivery of a product
to Midmark or one of its authorized dealers for warranty service, which
delivery shall be at retail purchaser’s expense. It is also the retail
purchaser’s obligation to comply with the warranty service instructions
provided either by Midmark or its authorized dealer. The retail purchaser must
provide Midmark with completed warranty registration information within thirty
(30) days after purchase in order to obtain the benefi ts of this warranty.
EXCLUSIONS
This warranty does not cover, and Midmark shall not be liable, for the
following:
- defects, damage or other conditions caused, in whole or in part, by misuse, abuse, negligence, alteration, accident, freight damage, tampering or failure to seek and obtain repair or replacement in a timely manner;
- products which are not installed, used, and properly cleaned and maintained as required in the Midmark “Installation” and/or “Installation/Operation Manual” for the applicable product;
- products considered to be of a consumable nature;
- accessories or parts not manufactured by Midmark;
- charges by anyone for adjustments, repairs, replacement parts, installation or other work performed upon or in connection with such products which are not expressly authorized in writing in advance by Midmark;
- costs and expenses of routine maintenance and cleaning; and
- representations and warranties made by any person or entity other than Midmark.
EXCLUSIVE REMEDY; CONSEQUENTIAL DAMAGES DISCLAIMER:
MIDMARK’S ONLY OBLIGATION UNDER THIS WARRANTY IS THE REPAIR OR REPLACEMENT OF
DEFECTIVE PARTS. MIDMARK SHALL NOT BE LIABLE FOR AND HEREBY DISCLAIMS ANY
DIRECT, SPECIAL, INDIRECT, INCIDENTAL, EXEMPLARY OR CONSEQUENTIAL DAMAGES OR
DELAYS, INCLUDING, BUT NOT LIMITED TO, DAMAGES FOR LOSS OF PROFITS OR INCOME,
LOSS OF USE, DOWNTIME, COVER AND EMPLOYEE OR INDEPENDENT CONTRACTOR WAGES,
PAYMENTS AND BENEFITS.
NO AUTHORIZATION
No person or fi rm is authorized to create or approve for Midmark any other
obligation or liability in connection with the products.
WARRANTY DISCLAIMER
THIS WARRANTY IS MIDMARK’S ONLY WARRANTY AND IS IN LIEU OF ALL OTHER
WARRANTIES, EXPRESS OR IMPLIED. MIDMARK MAKES NO IMPLIED WARRANTIES OF ANY
KIND INCLUDING ANY IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A
PARTICULAR PURPOSE. THIS WARRANTY IS LIMITED TO THE REPAIR OR REPLACEMENT OF
DEFECTIVE PARTS.
STATUTE OF LIMITATIONS
No action may be brought against Midmark for breach of this limited warranty,
an implied warranty, if any, or for any other claim arising out of or relating
to the products, more than ninety (90) days following expiration of the
limited warranty period.
