Product Information
This product is designed to provide a safe system of work for locking off machinery during maintenance or electrical tasks. It includes an isolator switch with a tag and padlock for ensuring the machine is securely locked off. The product also includes a lock-off box for storing keys and a lock off padlock logbook for record-keeping.
Product Usage Instructions
- Only trained personnel, such as Managers or Supervisors, should initiate the lock off process.
- If you are performing maintenance that requires removing safety guards or any electrical tasks, the machine needs to be locked off.
- Turn the isolator switch to the off position and either pull out or push in the tag at the end of the switch lever.
- Place a padlock through the tag and close it.
- Remove the key and store it in the lock-off box, which can be found on the door outside the Moulding/Engineering office. Use the provided DO NOT OPERATE label to mark the isolated switch.
- Fill out the lock off padlock logbook located on the Moulding/Engineering office door.
- When you are ready to release the isolator and return the padlock and key, sign the book with the date, time, and signature.
After receiving training, ensure that you wear the required personal protective equipment (PPE) when using the machinery and take all necessary steps to ensure your own safety. If you require further training or encounter any issues with the machinery, immediately inform your Supervisor or Manager.
Before using the machinery, sign the provided form to confirm that you have received training and fully understand the instructions. The authorized trainer will also sign the form to confirm your training and authorize your use of the machinery.

will be with Mark Oakley and Andy James. This will be situated on the door
outside the Moulding/Engineering office. Put the DO NOT OPERATE label on the
isolated switch.|
5| Fill out the lock off padlock logbook situated on Moulding/Engineering
office door.|
6| When you are ready to release the isolator and return the padlock and key,
you must sign the book with a date, time and signature.|
I have received the above training and fully understand every point that was explained to me. I confirm that I will wear the required PPE equipment when using the above machinery and take all steps to ensure my own safety. I will ask for further training if I need to and will immediately inform the Supervisor or Manager if there are any issues with the machinery.
