Intended Purpose

The Grip-Lok is intended to provide catheter securement.

Contraindications

None known.

Clinical Benefits Statement

The Grip-Lok permits securement of catheter to patient.

Warnings:

1. Sterile, Single use: Do not reuse, reprocess or resterilize.
   Reuse of device creates a potential risk of serious injury and/or infection which may lead to death. Reprocessing of medical devices intended for single use only may result in degraded performance or a loss of functionality.

2. Read all package insert warnings, precautions and instructions prior to use. Failure to do so may result in severe patient injury or death.

INSTRUCTIONS

1. Select the area for the placement of the Grip-Lok.
   Note: The catheter should be already inserted in the patient.

2. Prepare the skin according to the standard hospital protocol for dressing application. Hair removal may be required on some patients for better adhesion.

3. Open the top flap, remove the interior liner and hold the flap open.

4. Slide the Grip-Lok under the catheter and adhere in the exposed adhesive cut-out space provided.

5. Secure the top flap over the catheter.

6. Hold the Grip-Lok in position while removing the paper backing from one side, then the other, to secure on the skin surface.

Direction: for added skin protection, skin prep may be used prior to application of securement device.

Note: replace securement device if soiled or saturated in fluid or if device shows signs of wear or damage.

Note: use of an alcohol swab may help removal of securement device from the skin.
