TL-4016 Wireless Fall Prevention Monitor

CAUTION: If using this monitor with Change Pad Indicator™ Timed Sensor Pads, this monitor will not indicate when pad life has expired. Pads will still function normally. Call (800) 650-3637 for more information.
IMPORTANT INFORMATION: Cordless and Wireless Systems rely on wireless technology which is subject to physical and environmental considerations. Please be aware that the transmitter will not be 100% accurate if it is out of range at any time. It is the end-user’s responsibility to make sure that the product is used correctly and within range of each other. Cordless and wireless systems and devices are intended as an adjunct to good caregiving practices and are not a substitute for proper staffing and patient management practices. We recommend that all caregivers receive periodic training in the operation of these systems and that the devices are tested daily. It is important to read and follow the directions.

The Smart Caregiver Corporation devices are designed to be installed by the end user. As such, it is the entire responsibility of the buyer to insure that the system is properly installed and tested. Further, the system is not designed to replace good caregiving practices including, but not limited to:

A.) Direct patient supervision.
B.) Adequate training for staff personnel for fall prevention and elopement.
C.) Testing of the system before each use.

WARNING : The manufacturer does not claim that this device will stop elopement and/or stop falls. This device is designed to augment caregivers’ comprehensive resident mobility management program. Failure to comply with the warning may result in injury or death. This device is not suitable for all individuals. Other devices may be required. This device is not a substitute for visual monitoring by a caregiver.

Bed and Chair Sensor Pads

BED PLACEMENT
  • Place the pad under shoulders to be alerted when the resident sits up in bed.
  • Place pad under lower back/buttocks to be alerted when the resident vacates the bed.
  • Place the monitor out of reach of resident. Suitable mounting locations include: back of headboard, wall or under bed. Make sure the resident cannot tamper with monitor. (It is the caregiver’s responsibility to route the cord under the sheet so the resident cannot tamper with it).
WHEELCHAIR/CHAIR PLACEMENT
  • Center pad on the chair seat under buttocks.

  • Place the monitor out of reach of the resident. Suitable mounting location is the back of wheelchair, so that the resident cannot tamper with monitor.

IMPORTANT: TEST THE SYSTEM

  • Plug the pad into the monitor and apply pressure to the pad to activate the system. The Monitor will beep twice when activated.
  • The alert tone will sound when pressure is released from pad. Press the “Reset” button on front of monitor to silence the alert. The status light on the front of monitor will blink when pad is plugged into monitor and pressure is on pad.

Floor Mats

FLOOR MAT PLACEMENT
  • Place the floor mat in the desired location such as a critical doorway or next to a bed.

  • Place the monitor out of reach of resident. Suitable mounting locations include the back of the headboard, wall or under the bed so the resident cannot tamper with the monitor.

TEST THE SYSTEM
  • Step on Floor Mat to set off the alert. The monitor is now active. The alarm will sound whenever weight is on the floor mat.
  • When the alert is activated, press the “Reset” button on the front of the monitor to silence the alarm.

NOTE: The Status Light does not function when using floor mats.

Seat Belts

SEAT BELT PLACEMENT
  • Unscrew one screw on either side of wheelchair that holds seat in place.
  • Attach seat belt to wheelchair by lining up the grommet holes on seat belt with the screw holes. Re-insert screws through both seat belt and wheelchair to hold seat belt in place. Make sure the screws are tightened.
  • Adjust seat belt to desired length using the black buckle that holds the adjustment strap in place.
  •  Place the monitor out of reach of the resident. Suitable mounting location is the back of wheelchair, so that the resident cannot tamper with monitor.
TEST THE SYSTEM
  • Buckle and unbuckle the seat belt to set off the alert. The monitor is now active. The alert will sound whenever the seat belt is unbuckled.
  • When the alarm is activated, press the “Reset” button on front of the monitor to silence the alert. The status light on front of monitor will blink when the seat belt is plugged into the monitor and buckled.

Quick Start Instructions

Install a 3-AA Batteries

Using a small Phillips screwdriver, unscrew battery cover.
Insert a 3 new AA batteries as shown.
Optional:
Monitor can be used with AC Adapter (AC-05)
AC Adapter port is located on side of monitor.

*To silent monitor, move volume dial all the way to the left and be sure to use a pager or other caregiver alert option.

Connect a Sensor Device (Sensor Pad, Floor Mat, Nurse Call Button)

Sensor Pads, Floor Mats
Lace cord through the T-shaped strain relief to guard against accidentally unplugging the device.

Make sure the cord is placed so it is well out of resident’s reach and is not a tripping hazard.

Step on the floor mat. The alert tone will sound. Press the Reset button to silence  and reset the fall monitor.
Repeat steps 1–4 if system is not working properly

Mounting the TL-4016 Monitor

Mount the TL-4016 monitor within resident’s reach when using Nurse Call feature. Can be mounted out of resident’s reach with included hardware.

Instructions for Programming Monitor to Specific Number.

  1. Insert 2 of the AA batteries. Locate CALL and RESET on the front of the monitor.
    Press and hold CALL and RESET simultaneously and insert the third battery. You will hear 2 beeps within 5 seconds and the CALL button will light up solid blue to indicate you are in programming mode.

  2. This step is to program the area: The area is always defaulted to 01 unless you have specifically requested different areas on your products. (If you need to program a different area, you will enter that two-digit number during programming).
    a. Turn the dial to #0, press call. You wear hear one beep.
    b. Turn the dial to #1, press call. You will hear one beep.

  3. This step is for programming the room/bed number. You have four digits to program of your choice. For any digit not used you will need to program ‘0’. In this example we are programming number 123.
    a. Turn the dial to #0, press call. You will hear two beeps.
    b. Turn the dial to #1, press call. You will hear two beeps.
    c. Turn the dial to #2, press call. You will hear three beeps.
    d. Turn the dial to #3, press call. You will hear three beeps

  4. The blue light of the CALL button will turn off and the monitor is now programmed to the new number.

  5. Test the monitor with your Central Monitor and/or pager to confirm number has programmed successfully.
