Product Usage Instructions

1. On a plywood form, determine the fixture location using the paper template provided outside the pour cavity.
2. Knock out or drill an alternate hole in the box to mount wire conduit (1) and attach the conduit.
3. Drill through the corner 4 holes of the template with drywall screws (2) of sufficient length (not supplied) to capture the fixture box.
4. Pour concrete and allow it to cure.
5. Remove the corner screws (2) from the box and remove the plywood form.
6. Pull the wire through the conduit up to the fixture box.
7. Connect the fixture (3) following the fixture installation instructions.

Note: Measure the mounting template to make sure the dimensions on the template are correct.

1. On plywood form, determine fixture location using a paper template outside the pour cavity
2. Knock-out or drill alternate hole in box to mount wire conduit (1), attach conduit
3. Drill through corner 4 holes of the template with drywall screw (2) of sufficient length (not supplied) to capture the fixture box
4. Pour the concrete and allow the cure
5. Remove corner screws (2) from a box and remove plywood form
6. Pull wire through conduit up to fixture box
7. Connect fixture (3) as per fixture install instructions

Note: measure the mounting template to make sure dimensions on the template are correct.
