Product Usage Instructions

1. Before you begin, carefully read the Instructions for Use provided with the kit and follow the listed steps.
2. Ensure that all conditions for sample taking are fulfilled. Refer to the Information and tips section in the manual for further guidance.
3. Place all the test kit items on a clean surface and verify that none of the components are missing.
4. Open the packaging of the DNA sampling kit by following the opening arrow (“OPEN HERE”) at one end.
5. Gently pull out the sampling stick from the packaging using the handle. Ensure that the sponge head does not come into contact with any surface before sampling.
6. Insert the sponge head in front of your teeth, past the oral vestibule (not into the throat), and as deep as comfortable. Proceed with caution to avoid the risk of suffocation.
7. Rub the DNA swab against the inside of your cheeks, over your gums, and underneath your tongue for 60 SECONDS, similar to using a toothbrush.
8. To prevent leakage, hold the tube upright and unscrew the blue cap without touching the sponge. Place the sponge head into the tube and tightly close the lid.
9. Invert the sealed tube and vigorously shake it 15 times.
10. Remove the square code adhesive label from the test-ID card and affix it onto the filled DNA collection tube.
11. Slide the sample tube into the larger transport tube and ensure a secure seal.
12. Place the transport tube into the provided return envelope.
13. Seal the envelope carefully and send it immediately by post free of charge.
14. After completing the test, check if it has been activated as per the instructions. Dispose of the used parts of the test kit in the household waste.

IMPORTANT

• We can only analyse your sample if you activate your test with the test-ID of your test-ID card! More details on this can be found in step 2 + 3.
• The sample should only be taken between Sunday and Tuesday (latest) and sent off on the same day. Please take into account any public holidays.
• Before brushing your teeth, the sample should be collected.
• Please avoid the use of mouthrinses, mouthwashes, chewing gum or other substances refreshing your breath.
• Do not eat, drink and smoke 30 minutes before sample taking, as this may lead to the falsification of results.

WARNINGS

• Store this kit out of reach of children.
• This test is not suitable for children.
• The kit may only be used for adults aged 18 and older.
• The product is for single use only.
• Danger of suffocation.
• If eyes or skin come into contact with the preservative liquid, rinse them with water and contact a doctor.
• Do not ingest the liquid!
• If you are pregnant or are breastfeeding, use the kit only after consulting your therapist.

Directions for Use

1. Check that all conditions are fulfilled for sample taking. Please read carefully the section “Information and tips”.
2. If you are a new customer, create your user account on my.cerascreen.com or in the my cerascreen® App. In our ‘Frequently Asked Questions’ in help.cerascreen.com you will find a guide for that.
3. Log into the my cerascreen® app (Android, iOS) or my.cerascreen.com so that you can receive your test result. Go to Activate Test, enter the 6-digit ID number on your test ID card and then follow the instructions. Make sure that you activate the test on the sampling date.
4. Place all test kit items on a clean surface and check that none of the components are missing.
5. Carefully open the packaging of the DNA sampling kit from the end with the opening arrow („OPEN HERE“).
6. Pull the sampling stick out of the packaging by the handle. Make sure that the sponge head does not come into contact with any surface before sampling.
7. Insert the sponge head in front of the teeth past the oral vestibule (not into the throat) and as deep as comfortable. Proceed carefully to avoid the risk of suffocation.
8. Rub the DNA swab against the inside of your cheeks, over your gums and underneath your tongue for 60 SECONDS as you would use a toothbrush.
9. Now hold the tube upright to prevent the blue liquid from leaking out.
10. Unscrew the blue cap from the tube without touching the sponge. Put the sponge head into the tube and close the lid tightly.
11. Turn the sealed tube over and shake it vigorously 15 times.
12. Remove the square code adhesive label from the test-ID card and stick it on the filled DNA collection tube.
13. Then slide the sample tube into the larger transport tube and seal it well. Place the transport tube in the return envelope. Close the envelope carefully and send it immediately by post free of charge. Please check if the test has been activated and dispose of the used parts of the test kit in the household waste.
