PACIFIC Our Kitten and Puppy Plans

Product Information

The Pacific Veterinary Hospital offers various membership plans for kittens, puppies, and adult pets. These plans provide discounted services and savings on veterinary care.

Kitten and Puppy Plans
Membership Fees:

Restrictions:

• Plan is non-transferable.

• Plan cannot be applied to past visits.

• An appointment must be scheduled for services to be used.

• Additional discounts apply to eligible non-plan items only and
  cannot be used with other promotions and/or discounts.

• Additional costs may apply to nail trims if sedation is required.

• If the patient is in heat, pregnant, or cryptorchid during surgery, additional fees will apply.

Adult Plans
Service Guide:

• $35.00 examination copay (unlimited)
• Technician office visits (unlimited)
• Complimentary level 1 nail trims (unlimited)
• Biannual preventive care examinations
• OSHA/biohazard fee (unlimited)
• Core vaccine boosters (administered based on age, breed, and risk)
• Heartworm and tick-borne parasite screen (dogs)
• Annual comprehensive wellness screen:
  • Cats: CBC, blood chemistry, electrolytes, Total T4, fecal, Feline Leukemia and FIV test
  • Dogs: CBC, blood chemistry, electrolytes, Total T4, fecal, and urinalysis
• Pre-anesthetic ECG screen
• Routine dental cleaning package
• Thyroid T4 screen
• Radiographs (3 views)
• Radiograph consult
• Blood pressure screen

Product Usage Instructions

To use the Pacific Veterinary Hospital’s membership plans, follow these instructions:

1. Select the appropriate plan based on whether you have a kitten, puppy, or adult pet.
2. Pay the initial membership fee for the chosen plan.
3. Make monthly payments as specified for kittens, puppies, cats, or dogs.
4. To benefit from the plan, schedule an appointment for the desired services.
5. Remember that additional discounts only apply to eligible non-plan items and cannot be combined with other promotions or discounts.
6. If your pet requires sedation for nail trims or has special conditions during surgery, additional fees may apply.
7. For adult plans, take advantage of the unlimited examination copay, technician office visits, and complimentary level 1 nail trims.
8. Ensure biannual preventive care examinations are scheduled for your adult pet.
9. Follow the recommended core vaccine boosters based on your pet’s age, breed, and risk factors.
10. Dogs should undergo a heartworm and tick-borne parasite screen annually.
11. Cats and dogs will receive comprehensive wellness screens, including various tests and screenings.
12. During dental cleanings, extractions, medications, and other charges will be incurred separately.
