Product Information
The WS-68227/WS-68237 is a product designed by Modern Forms for use as a fixture in homes or offices. It comes with all the necessary parts and accessories needed for installation, and it is important to use only the parts designated for use by Modern Forms. Substituting parts or accessories not designated for use with this product could result in personal injury or property damage, and will void the warranty. Customers living in coastal areas are advised to clean all external surfaces of the fixture once every two weeks with a wet cloth to avoid mineral and residue build-up.
Product Usage Instructions
- Shut off the power at the circuit breaker and remove the existing fixture, including the crossbar.
- Carefully unpack the new fixture and lay out all the parts on a clear area. Be careful not to lose any small parts necessary for installation.
- Remove the mounting screw (B) from the fixture and take down the mounting back plate.
- The lamp can switch between three color temperatures (2700K-3000K-3500K), which can be adjusted to your preference with the switch in the canopy (Fig.4).
- Drill holes in the wall aligned with the keyholes located in the mounting plate, insert the plastic anchors (H).
- Secure the mounting plate to the switch box using assembly screws (C). Fasten the back plate to the wall with the plastic anchors (G) using the wood screws (H) provided. Note: (i) Using any other non-original-manufacturer-provided junction box screw may result in safety issues; (ii) The side of the mounting plate marked GND must face out.
- Connect the fixture’s wires to junction box wires as shown in Fig. 3, making sure that all wire connectors (A) are secured. If your outlet box has a green and bare copper ground wire, connect the fixture’s ground wire to it. Otherwise, connect the fixture’ ground wire directly to back plate using the GND screw provided. After wires are connected, tuck them carefully inside the switch box.
- Secure the fixture to the mounting back plate using the mounting screws (B).
For mounting on 4×4 junction boxes, follow the instructions provided in Fig.12.
CAUTION: To prevent electrical shock, ensure electricity has been turned off at the circuit breaker before beginning.
- Read all instructions before installing.
- System is intended for installation by a licensed electrician in accordance with the National Electrical Code (NEC) and local regulations.
- When handling the fixture, do not apply pressure to the LEDs. Hold the fixture by the base only.
- Retain installation instructions for future maintenance reference.

Contact an authorized dealer or the manufacturer if any parts are damaged or missing.
MAINTENANCE NOTE: Coastal conditions may cause mineral and residue build- up on the fixture. We recommend that customers in coastal areas clean all external surfaces of the fixture once every two weeks with a wet cloth.

PREPARATION

MOUNT ON 2” X4” SWITCH BOX -
Remove the mounting screw (B) from the fixture, and then take down the mounting back plate.

Backplate Dimensions
MOUNT ON 4”X4” JUNCTION BOX
- Remove the mounting screw (B) from the fixture, and then take down the mounting back plate.

- Drill holes in the wall aligned with the key holes located in the mounting plate, insert the plastic anchors (G).
- Secure the mounting ring (D) to the junction box using junction box screws.The side of the mounting ring marked “GND” must face out.
- Secure the mounting back plate and conversion plate (E) to the mounting ring (D) with the Assembly screws (C).
- Secure the mounting plate to the junction box using Assembly screws (C). Fasten the back plate to the wall with the plastic anchors (G) and spacers (H) using the wood screws (F) provided.
- Connect the fixture’s wires to junction box wires as shown in Fig. 3, making sure that all wire connectors (A) are secured. If your outlet box has a green and bare copper ground wire, connect the fixture’s ground wire to it. Otherwise, connect the fixture’ ground wire directly to back plate using the GND screw provided. After wires are connected, tuck them carefully inside the junction box.
