SUBJECT: Tail Rotor Crosshead Duplex Bearing and Counterweight
Installation
This OSN is to remind 214ST owners and operators about the importance of
proper assembly of the parts associated with the tail rotor mast and pitch
change mechanism. An incident occurred recently that resulted in fracture of
the end of the mast, and the most
likely cause is improper orientation of the crosshead bearing. If the bearing
is not installed correctly it can lead to higher dynamic loads in the mast and
subsequent failure. Improper positioning of the counterbalance weights will
also result in higher than desired loads.
Users are reminded that the orientation of the crosshead duplex bearing and
configuration of the counterweights are critical to avoid failure of the tail
rotor mast. If there is any doubt about the proper installation of these
items, users are encouraged to verify their proper installation.
