Product Usage Instructions
- Log on to anthem.com using a web browser.
- Enter your user ID and password to access the platform.
- If it’s your first time logging in, select “Register Now” to create a user ID and password.
- Once logged in, navigate to Claims & Payments and then to the Explanation of Benefits Center.
- Select “Filter EOB” and enter the desired timeframe to include relevant transactions.
- Identify the EOB that corresponds to the unsubstantiated claim based on date and member responsibility.
- Download the EOB document for further processing or upload to spending accounts.
- Save the document for future reference or upload it to spending account transactions.
Obtain EOBs or Other Documentation
Medical EOBs for yourself and those covered by Anthem are available online at
Anthem.com. If you used your spending account for expenses of a family member
who is not covered by Anthem medical, logon to the site of that health plan
and obtain the Explanation of Benefit (EOB) for applicable timeframe and upload
using the instructions below. If you used your spending account for dental or
vision expenses, logon to Aetna Dental or EyeMed.
You can access a medical EOB at anthem.com as follows:
Login to anthem.com
Enter your user ID and password.
If you have not previously logged on, you need to select Register Now to create a user ID and password.
Select Claims & Payments, then Explanation of Benefits Center.
Select Filter EOB and enter the appropriate timeframe to include the timeframe with unsubstantiated transactions as identified in the pending/processing transactions.
Explanation of Benefits (EOBs) will display. Select the EOB relevant (in terms of date and member responsibility) to the unsubstantiated claim. Download it for upload to the spending accounts.
Save the document for upload to the spending account transaction.
