BioTel Heart ePatch Extended Holter Monitor Instructions
Step 1
Important : Do not throw the box away. You will need to use it to return the equipment and supplies.
Step 2
When you are ready to return ePatch, pack up the sensor, unused patch pouches, diary and any other supplies, and place into the kit.
Step 3
Remove the protective strip to expose the adhesive. Seal the kit shut. Return the pre-paid sealed kit back to BioTel Heart via UPS. You can find the nearest available location by going towww.ups.com/dropoff. There is no cost to you to mail back the equipment. If you are unable to bring ePatch to a UPS drop off location, please call 1-877-593-6421 for instructions on how to schedule a pick up. Remember to mention that you have a UPS pre-paid label.
