Instructions

1. Print and fill out sections 1, 2, and 3 of this form
2. Make sure sandals are clean, fully dry, and free of debris
3.  Ship to Bedrock at 101 S. California St, STE B Missoula, MT 59801
4. Upon receipt we’ll assess the work and send an electronic invoice
5. Re-Souls and Repairs average 2-3 weeks to complete
6.  Upon completion and payment, Bedrock will ship back sandals to the address listed on this form. US repairs ship via USPS priority and are charged a flat rate $8 fee. International repairs ship via economical service and are charged $25-$35 dollars depending on the region
