This is what is in the kit:
EXCITE COVID-19 EL Sampling Kit
IMPORTANT:
| Your sample should be dropped off on the same day it was collected. Your
sample MUST NOT be frozen.

| You will need to download the “UCSD mobile app” from the App Store (iPhone)
or Google Play (Android).**
| Your results will be sent to your MyChart account in the UCSD EPIC
Electronic Health Record.
| If your results are not back within 48 hours, please submit another sample
and email
studenthealth@ucsd.edu.
This kit may be used for sample self-collection by persons 15 years of age and over. For children between 2 and 14 years of age, samples are collected by an adult. For children younger than 2 years of age, samples are collected by a Health Care Provider.

Use hand sanitizer.

Pick up the tube.

Scan tube barcode using the UCSD mobile app on your phone or iPad.**

Make sure the barcode number on the top of the apps’s confirmation page matches your tube.

Peel open swab package.

Insert swab tip 1 cm (1/2 inch) into one nostril. Rotate tip against septum 3 times. Hold in place 15 seconds.

Repeat with other nostril.

While holding the swab, unscrew tube lid. Drop the swab into the tube.

Screw on tube lid firmly.

Transport the tube to a dropbox. Your sample must be returned on the same day that the sampleis collected.

Drop the your sample into the dropbox.

Use hand sanitizer.
* For use under Emergency Use Authorization only.
- This product (collection kit in combination with the authorized test) has not been FDA cleared or approved, but has been authorized for emergency use by FDA under an EUA for use by the authorized laboratory;
- This product has been authorized only for the detection of nucleic acid from SARS-CoV-2, not for any other viruses or pathogens; and
- The emergency use of this product is only authorized for the duration of the declaration that circumstances exist justifying the authorization of emergency use of in vitro diagnostics for detection and/or diagnosis of COVID-19 under Section 564(b)(1) of the Federal Food, Drug, and Cosmetic Act, 21 U.S.C.
§ 360bbb-3(b)(1), unless the declaration is terminated or authorization is revoked sooner.
