Product Usage Instructions

Collect the Devices for Return
Ensure you have all the necessary components ready for return.

Power Off the VistaPhone
To power off the VistaPhone:

  1. Press and hold the Power Key on the right side of the phone until the Options menu displays.
  2. Select the power-off option from the menu.

Place Devices in Return Box

Follow these steps to pack the devices:

  1. Place the phone and case in the middle slot of the box.
  2. Put the USB charger and charge cable in the lower slot.
  3. Insert any unused VitalPatch devices into the top slot.

Seal Box Closed
Seal the box securely using provided stickers or tape.

If you have a return label:
For USPS Return Label:

  1. Place the return box in the shipping box or envelope.
  2. Ensure the USPS return label is visible.
  3. Keep the tracking number for reference.

For UPS Return Label:

  1. Put the return box in the shipping box or envelope.
  2. Confirm the UPS return label is visible.
  3. Contact UPS for pick-up if needed.
  4. Retain the tracking number from the return label.

You must return the devices when monitoring is complete. Failure to return may delay test results and you will receive a bill for the cost of the device.

Collect the Devices for Return

  1. Find the following items:
  2. VistaPhone, charger, and charge cable
  3. Unused VitalPatch biosensors (in their original, undamaged pouches)
  4. The original shipping box (to be used for return).
  5. Discard the following items that were provided to you:
  6. Used VitalPatch biosensors (follow local laws for disposal of routine electronic waste)
  7. Alcohol wipes, adhesive removal pads, shaving razors and instruction sheets

Place Devices in Return Box

Return to VitalConnect or Doctor’s Office

Return to Doctor’s Office
If you were not provided a return label, return the box and equipment to the location requested by your healthcare provider (for example, your doctor’s office).

If you have a US Postal Service (USPS) Return Label
If you were provided an envelope or shipping box with a pre-paid US Postal Service (USPS) return label, you must use USPS to return the equipment to VitalConnect:

  1. Place the return box into the shipping box or envelope and seal closed using provided stickers or tape.
  2. Check that the USPS return label is visible.
  3. Drop off at any post office or secure mailbox.
  4. Save the tracking number from the return label for your records.

If you have a UPS Return Label
If you were provided an envelope or shipping box with a pre-paid UPS return label, you must use UPS to return the equipment to VitalConnect:

  1. Place the return box into the shipping box or envelope and seal closed using provided stickers or tape.
  2. Check that the UPS return label is visible.
  3. Drop off at any UPS shipping location. To schedule a pick-up, contact UPS at 1-800-742-5877. Inform UPS that you have a pre-paid label. Do not use the post office or mailbox to return.
  4. Save the tracking number from the return label for your records.
