Please fill out this form on your computer and print it out. Or you can print it out and fill it out by hand. Add it to the box you return the pager in. A return merchandise authorization (RMA) is not needed to conduct a repair. Please send in pagers without belt clips/ battery covers and batteries. The warranty does not cover defects, malfunctions, performance failures or damages to the unit resulting from use in other than its normal and customary manner; misuse, accident or neglect, the use of non-conforming parts, or improper alterations or repairs. This warranty does not cover batteries, battery chargers, housings or holsters, wear and tear on covers and coverage or range over which the pager will receive signals. Anything out of warranty will cost $50 for Digital and $75 for Voice and Hurricane Duo Pager. Cost for reprogramming or re-tuning to a new frequency is $10. Cost for Firmware Upgrade is $25. Freight cost is not included.
