Product Information

The FreeStyle Libre 3 Continuous Glucose Monitoring System is a device that helps monitor glucose levels in individuals. It consists of a Reader and a Sensor Applicator.

Reader Features:

Sensor Applicator Features:

• Tamper Label for product integrity
• Cap to protect the Sensor

The system provides accurate glucose readings when used correctly. It is important to follow the instructions provided in the User’s Manual for optimal results and to ensure the system functions properly.

Product Usage Instructions

Step 1: Apply Sensor to the back of your upper arm

1. Select a site on the back of your upper arm. Avoid scars, moles, stretch marks, lumps, and insulin injection sites.
2. Wash the site using plain soap, then dry it.
3. Clean the site with an alcohol wipe and allow it to air dry.
4. Unscrew the cap from the Sensor Applicator.
5. Place the Sensor Applicator over the prepared site and push down firmly to apply the Sensor. Do not push down until the Sensor Applicator is placed over the site to prevent unintended results or injury.
6. Gently pull the Sensor Applicator away from your body, ensuring the Sensor is secure.
7. Put the cap back on the Sensor Applicator and discard the used Sensor Applicator according to local regulations.

Step 2: Start new Sensor with Reader

1. Press the Home Button on the Reader to turn it on.
2. If using the Reader for the first time, follow the prompts to set it up.
3. Touch “Start New Sensor” when prompted.
4. Scan the Sensor using the Reader by holding it near the Sensor. Move the Reader around slowly until you find the right spot.
5. Important: Before starting the Sensor, choose which device you want to use. If you start the Sensor with the Reader, you will be unable to use the App to check your glucose or receive alarms.
6. Review the important information on the screen and touch “OK”.
7. The Sensor will start up and can be used after 60 minutes.

Step 3: Check your glucose

1. Press the Home Button on the Reader to turn it on.
2. Touch “View Glucose” from the Home Screen.
3. Note: The Reader automatically gets glucose readings when it is within 33 feet of your Sensor.
4. The Reader will display your glucose reading, including your Current Glucose, Glucose Trend Arrow, and Glucose Graph.

Setting Alarms:
The Sensor can give you glucose alarms, which are on by default. To change their settings or turn alarms off, follow these steps:

1. Refer to your User’s Manual for detailed instructions on how to access the alarm settings.

1. Apply Sensor to the back of your upper arm
2. Start new Sensor with Reader
   • Wait 60 minutes for start-up
3. After the start-up period, you can use the Reader to check your glucose

Apply Sensor to the back of your upper arm

STEP 1
Select site on back of upper arm. Do not use other sites as these are not approved and may result in inaccurate glucose readings.

Note: Avoid scars, moles, stretch marks, lumps, and insulin injection sites. To prevent skin irritation, rotate sites between applications.

STEP 2
Wash site using a plain soap, dry, and then clean with alcohol wipe. Allow site to air dry before proceeding.

STEP 3
Unscrew cap from Sensor Applicator.

CAUTION:

• Do NOT use if damaged or if tamper label indicates Sensor Applicator has already been opened.
• Do NOT put cap back on as it may damage the Sensor.
• Do NOT touch inside Sensor Applicator as it contains a needle.

STEP 4
Place Sensor Applicator over site and push down firmly to apply Sensor.

CAUTION:
Do not push down on Sensor Applicator until placed over prepared site to prevent unintended results or injury.

STEP 5
Gently pull Sensor Applicator away from your body.

STEP 6
Make sure Sensor is secure. Put the cap back on the Sensor Applicator. Discard used Sensor Applicator according to local regulations.

Start new Sensor with Reader

STEP 1
Press Home Button to turn on Reader. If using Reader for the first time, follow the prompts to set up the Reader. Then touch Start New Sensor when you see this screen.

STEP 2
Hold the Reader near the Sensor to start it. You may need to move your Reader around slowly until you find the right spot.

Note:
Before you start your Sensor, choose which device you want to use. If you start the Sensor with the Reader, you will be unable to use the App to check your glucose or receive alarms.

STEP 3
Review the important information on the screen. Reader will automatically display your glucose reading after 60 minutes.

Check your glucose

STEP 1
Press Home Button to turn on Reader and touch View Glucose from Home Screen.

Note:
Reader automatically gets glucose readings when it is within 33 feet of your Sensor.

STEP 2
Reader shows your glucose reading. This includes your Current Glucose, Glucose Trend Arrow, and Glucose Graph.

Setting Alarms

• The Sensor automatically communicates with the Reader and can give you glucose alarms.
• Alarms are on by default. To change their settings or turn alarms off, follow these steps.

IMPORTANT:
Glucose alarms are an important safety feature. Please consult your health care professional before making changes.

STEP 1
Press Home Button to go to Home Screen. Touch.

STEP 2
Touch Alarms and then touch Change Alarm Settings.

STEP 3
Select and set your alarms. Touch done to save.

Using Alarms

Touch Dismiss Alarm or press the Home Button to dismiss the alarm.
