Important Message
As manager of Quebec’s identification and traceability system, Attestra must,
on behalf of MAPAQ, collect all the necessary information for the
identification and traceability system. This information is necessary to
become registered with Attestra and to open your file which will allow you to
place your identifier orders, make your traceability declarations with regard
to the Regulation respecting the identification and traceability of certain
animals (P-42), consult your animal inventory, etc.
Your personal information is important to Attestra. For this reason, Attestra
wishes to inform you that the personal information collected in the enrolment
form will remain confidential, in accordance with the provisions of the Act
respecting access to documents held by public bodies and the protection of
personal information.
Also, you are responsible for updating the information provided to Attestra.
You have the right to refuse to provide certain information. However, in such
a case, your file cannot be opened.
Instructions for Filling Out the Registration Form
The following instructions provide more details on certain aspects of the
registration form in order to assist you in filling it out. Should you need
additional information, we would invite you to contact Attestra’s Customer
Service from Monday to Friday, from 8:30 a.m. to 12:00 p.m. and from 1:00 p.m.
to 4:30 p.m., at the following number: 1 866 270-4319.
Part 1. Enterprise Information
1.1 Enterprise Information
Legal name: This is the name that has been legally registered with the
Registraire des Entreprises for general partnerships, limited partnerships,
companies, cooperatives and associations. For an individual or a joint venture
whose name does not appear in the registry for enterprises, the last and first
name of the individual or individuals who own the company must be entered.

Legal status: In Quebec, any and all enterprises operate in a legal form.
Please indicate your enterprise’s legal form.

Quebec Enterprise Number (NEQ): The NEQ is a unique numerical identifier that
is assigned to each enterprise registered in the enterprise register. Only an
individual and certain joint ventures are not required to appear in the
enterprise registry.

Mailing address for correspondence: You must enter the postal address where
you wish to receive mail from Attestra.
E-mail address for correspondence: If it is possible to communicate with you
by e-mail, enter your company’s main e-mail address (the one you use most
regularly). Please note that by giving your e-mail address, you authorize
Attestra to communicate with you by e-mail.
An e-mail address must contain a “@” character to be valid.
Online account (SimpliTRACE): An e-mail address is required to access your
online account. Please provide the e-mail address that will be used to access
your account. Following the opening of your file with Attestra, a temporary
password and instructions for activating your account will be sent to you by
e-mail.
1.2 Information on the Representatives for the Livestock Operation
The representatives for the livestock operation are the individuals that
Attestra will be in contact with regarding updating the file, traceability
declarations, animal inventory, identifier orders or any other information
related to the animal identification and
traceability system.
Main representative: The individual implicated by the regulation who has all
the rights related to the file. This person is the account administrator and
can authorize secondary representatives. This person is responsible for the
rights granted to those designated as secondary representatives.
Secondary representatives: Any other person identified by the main
representative and having the rights as assigned by the main representative.
Position: The representative’s relationship to the enterprise (ex: owner,
shareholder, spouse, etc.).
1.3 Alternate Identification
The alternate identification corresponds to the identification your enterprise
uses when communicating with other organizations.
This information is optional.
Part 2. Enterprise Characteristics
2.1 Livestock
You must indicate all livestock owned by your enterprise. Please indicate the
main category and the subcategories. For example, Bovine and fed cattle or
Ovine and light lamb.
In section 2.3, the livestock raised on each of your premises must be
indicated.
2.2 Permit Types and Numbers | Cervids only
The ownership of cervids is governed by the Regulation respecting animals in
captivity, and the keeping of these animals requires a permit.
If you own cervids, you must indicate the type of permit, as well as the
permit number. You can find information about the types of permits on the
website of the ministère des Forêts, de la Faune et des Parcs at the following
address: https://mffp.gouv.qc.ca/la-faune/captivite/reglementation-permis
/permis-formulaires/ (French only).
Types of permits:
- Professional permit for the keeping of animals on a game and hunting farm or for deer farming: this type of permit is issued to agricultural enterprises that keep animals for breeding purposes or for those that offer hunting in an enclosed area;
- Professional permit to keep animals: this type of permit is issued to enterprises that keep animals for educational and entertainment purposes such as a zoo;
- General permit to keep animals: this type of permit is issued to individuals that own these animals for recreational purposes;
- Permit to keep native animals in rehabilitation: this permit authorizes the keeping in captivity of injured or orphaned animals found in the wild in Quebec for rehabilitation purposes.
2.3 Livestock Location (Premises)
Premises are a parcel of land defined by a legal description (cadastral lot)
or, in its absence, by georeferenced coordinates, on which animals are kept,
assembled or disposed of. Each premises are identified by an official number
which is required for traceability information declaration purposes. Only one
identification number may be associated with the premises even if several
species are present on the premises. You must identify each of your premises
that correspond to a cadastral lot on which bovines, ovines or cervids are
raised and enter the corresponding information.
Description: The description of the premises corresponds to the name
given by the stakeholder to better identify the place where the animals are
located and thus facilitate correspondence between the number that will be
allocated and the place. For example, sheep barn behind the house, red barn,
etc.
Type: The type corresponds to the type of building or premises. For
example, building with pasture, sheep barn, etc.
Capacity: The capacity corresponds to the maximum number of animals that
can be kept on the premises at one time.
Cadastre number (lot): This information is necessary to assign a number
to the premises, which is defined by a legal description (cadastral lot) and
serves to validate the information. The cadastral number is found on the
property tax assessment notice.
Registration number: This information is necessary to assign a number to

Livestock raised on the premises: For each of your premises, you must
indicate the specific livestock raised on these premises.
Part 3. Signature and declaration
It is mandatory to write your last name and first name in block letters, to
print all the pages and then sign and date the form.
When completed, please return the form by mail, fax or e-mail to the contact
information listed on page 5.
Attestra reserves all property rights. Full or partial reproduction,
electronic transmission or by any other form or means, content modification,
use for commercial purposes or public release are prohibited without
Attestra’s written consent.
