deserve and the workers’ compensation benefits you may be entitled to receive.
Promptly report your injury.
Time is of the essence: It’s best to report your injury to your
supervisor within 24 hours. In general, you should report your injury as soon
as possible to qualify for workers’ compensation benefits.
Get medical guidance.

Submit a claim checklist.
Be prepared with the following information when you go to file a claim:
· Employer’s name and address.
· Your name, address, social security number, and date of birth.
· State where you work.
· Your occupation (job title) at the time of injury.
· Date of injury.
· Address/location of accident (was the accident on employer’s premises?).
· How the accident occurred.
· How many people were injured in the accident.
· Part(s) of the body injured.
· If you returned to the next scheduled shift after the accident.
· Your wage on the date of injury (salary per hour, day, week, or month).
