Attach a copy of at least one current employee EPA
Certification Card 608 or 609. -
Email the form to sales@nyhvacsupplyhouse.com or mail it to the
address mentioned.
Certification Options:
Choose one of the following options that best applies to
you:

Certified Technician: If you are a certified technician,
enclose a copy of your EPA 608 Certification and/or 609
Certification card. -
Company: If your company employs certified technicians, enclose
a copy of at least one current employee’s certification
identification card. -
Wholesaler Resale: No certification needed, but ensure
compliance with refrigerant sales restrictions.
Future Notifications:
If there are changes in regulations, NY HVAC SUPPLY HOUSE LLC
will notify you prior to future refrigerant purchases.

Instructions: Return this form via email or mail with a copy of at least ONE
current employee EPA Certification Card 608 or 609.
I here notify NY HVAC SUPPLY HOUSE LLC that our company/ or I are/ am eligible by law to purchase Class I, Class II and other regulated refrigerants because:

___ Certified Technician- I am a technician who has been certified in
accordance with EPA for 608 Certification and/or 609 Certification under
regulation 40 CFR 82 subpart F. (HVAC-equipment) or 40 CFR 82 subpart B (motor
vehicles). I have enclosed a copy of EPA 608 Certification and/or 609
Certification card.
___ Company- We employ at least one technician who has been certified in accordance with EPA for 608 Certification and/or 609 Certification under regulation 40 CFR 82 subpart F. (HVAC-equipment) or 40 CFR 82 subpart B (motor vehicles). We have enclosed a copy of at least one current employees certification identification card.
___ Wholesaler Resale- No certification needed for resale of refrigerant. But you must ensure compliance with the refrigerant sales restriction, EPA recommends that you keep sufficient records to establish that the purchaser of the refrigerant is going to resell it to certified technicians.
In the event regulations change, we will notify you prior to future purchases of refrigerants.
