Product Description:

The Madaco Safety Products roof anchor RF-1034 is a heavy-duty gauge steel bracket designed for temporary installation on wood frame flat or sloped roofs. It meets industrial standards and should be used after adequate employee training.

Product Usage Instructions

Applications:

Fall Arrest Application: The roof anchor is part of a fall arrest system, including a full-body harness, lifeline, shock absorber, rope grab, and connecting devices. The maximum allowable fall distance per OSHA standards is 6 feet.

Restraint Application: The roof anchor is part of a restraint system with a full-body harness and non-shock-absorbing lanyard to prevent the user from reaching hazards.

Precautions and Limits:

Always have a certified fall protection specialist assess the work environment before using the equipment. Use only MSP approved components to maintain performance and warranty.

Compatibility:

Use only MSP approved components or subsystems with the equipment. Substituting non-approved items may void the warranty and alter equipment performance.

Anchor Strength Requirements:

Each roof anchor must be attached to a roof member capable of supporting at least 5,000 lbs (22.2 kN) for fall arrest applications.

DESCRIPTION

RF-1034 is a heavy duty gauge steel bracket designed to be use as a temporarily installed anchor point on the wood frame located on flat or sloped roofs. Minimum breaking strength, 5,000 lbs. (22.2 kN) During use the steel anchor plates are attached to the roof structure, and the O-ring is used for connection of fall restraint or arrest systems.

APPLICATIONS

Model Number: RF-1034 roof anchor is intended to be used as a temporarily installed anchor for use on wood frame structures. This product is not intended to be used as a permanently installed system. RF-1034 roof anchor can be used as an anchor to the MSP’s Tuff-Shox Horizontal Lifeline System. This product is designed with the capacity of one person, the weight of the person shall not exceed 310 lbs including tools. See Figure 2 for application illustration. Do not hang, lift, support tools or equipment from this roof anchor. Do not use any other types of lines to attach on to the roof anchor besides designated fall protection, restraint rope or lanyard system equipment.

A. FALL ARREST APPLICATION:

The roof anchor is used as part of a complete fall arrest system. Such systems typically include a full body harness, life line, shock absorber, rope grab and connecting devices (eg. Carabiners, Snap hooks). Per OSHA standards the maximum allowable fall distance is 6 feet. This type of fall arrest system is required on both commercial and residential roofing projects when working at a height of over six feet regardless of the pitch of the roof.

B. RESTRAINT APPLICATION:

The roof anchor is used as part of a complete restraint system. Such systems typically include a full body harness, connecting devices and a non shock absorbing lanyard used to restrain the user from reaching a hazard (eg. leading edge roof work).

PRECAUTIONS AND LIMITS OF USAGE FOR THIS PRODUCT:

Always have a certified fall protection specialist to assess the work environment and application prior to use of this equipment.

COMPATIBILITY OF COMPONENTS AND SUBSYSTEMS:

This equipment is designed for use with MSP-approved components or subsystems (Shock-absorbing lanyard, self retractable lanyard, harnesses and etc.). Substitutions or replacement of equipment with non-approved components or subsystems may alter the performance of the equipment. The use of any substituted non-approved items will void the warranty. MSP will not be liable for any damages as a result of misuse of any fall protection components. Guidelines set-forth in accordance to ANSI A10.14. , ANSI Z359.1-2007. Please contact MSP for questions on the compatibility of the equipment or components of the system.

Anchor strength requirement for FALL ARREST applications:

During installation, each roof anchor installed for fall arrest applications must be attached to a roof member capable of sustaining static loads in the direction(s) permitted by the PFAS when in use of at least: 5,000 lbs. (22.2kN). If multiple roof anchors are installed onto a roof structure, each anchor location must be independently capable of supporting 5,000 lbs.

  • Model: RK-50 is a PFAS that has a full body harness attached to the

L-RG-50 lifeline system with which is connected to the roof anchor via a carabiner. When used properly the Roof kit will arrest a fall and limit the arresting forces to less than 900 LBS.

Guidelines and limitations:

  1. Be aware of the working environment; do not work near moving machines.
  2. Be sure all connecting components are compatible in shape, size, and strength.
  3. Do not alter the product in any way, alteration voids all warranty. Only a certified engineer from Madaco is allowed to make alterations to the product.
  4. User must have a rescue plan in place prior to installing this product.
  5. Always inspect the rope/shock pack/rope grab for previous deployment. If found, mark the product unusable and destroy the product immediately.
  6. Do not use Synthetic rope after contact with chemicals.
  7. Rope must be kept dry and store in mild temperature room- 62 ~ 80 degrees Fahrenheit.
  8. Rope Grab must be free or corrosion and rust.
  9. Use only compatible components.
  10. Make sure no obstruction is below the user.
  11. Avoid contact with electrical hazard and chemical corrosion.
  12. Avoid sharp edges.
  13. Make sure all connecting hardware is compatible & meets the ANSI Z359.1 standard.

Remark:

Maximum free fall distance can not be greater than 6 ft, (FED OSHA) (Cal –OSHA). One must ensure adequate clearance in the fall path to avoid striking objects below during a fall. Shock absorber elongates to 42 inches, keeping arrest force under 1800 lbs. Always calculate and understand the safe free fall distance needed prior to installation and using the product. To limit free fall distance, user can manually adjust the height of the rope grab during descent. Try and work directly below the anchorage as possible to a avoid swing fall hazard. Always keep rope grab position as high on the rope overhead to limit free fall distance. When descending on the rope grab, press or squeeze on the lever of the rope grab to descend. To stop descending, release the lever of the rope grab.

Anchorage Requirement:
Anchor strength requirement when using product for FALL ARREST
applications: During installation, each roof anchor installed for fall
arrest applications must be attached to a roof member capable of
sustaining static loads in the direction(s) permitted by the PFAS
when in use of at least 5,000 lbs. (22.2kN). Anchorage used for
attachment must be independent of any anchorage used to support or
suspend platforms. Anchor must support 5,000 lbs per user.

INSPECTION AND MAINTENANCE

  1. Inspect rope grab device-body, lever system and connecting hardware, these items must function and be free of defect, distortion, rusts, corrosion, crack and worn parts.
  2. Inspect the entire length of the Rope, check for frayed, splinters, discoloration, mold, cuts, burns, broken stitches.
  3. All labels must be present. If any labels are missing from any components of the RK-50 roof kit, refrain from using the equipment until labels have been replaced.
  4. Shock pack must be inspected for signs of deployment or activation of the shock absorber. If the inner webbing of the shock reveals white colored webbing, the unit has been impact loaded. User must immediately remove the product from service and to be destroyed by cutting the webbing, and mark “Unusable”!
  5. Store equipment in cool and dry, clean indoor location, avoid direct sunlight. Avoid areas of exposure to liquid or vapors from chemicals and acids.
  6. Clean shock absorbing lanyard and rope with water and mild soap solution.
  7. Do not use bleach or other chemicals to clean the webbing.
  8. Dry hardware with soft cloth, hang to air dry. Do not machine dry.
  9. Daily check for excessive build up of dirt, oil, paint, etc. on the rope grab.
  10. Always clean up excessive build up, to ensure safe and proper Rope Grab performance.
  11. It is the user’s responsibility to inspect before, during and after each use on the rope grab mechanism. Failure to follow routine inspection could lead to serious injury or death.
  12. Always, inspect your equipment to ensure safe performance.

Safe Operation and Usage

  1. Anchorage point selected for anchoring must be capable of sustaining static loads of 5000 lbs. (Consult a competent person for proper assessment of the anchor point.
  2. Install Roof anchor to the proper anchor objects or points by following the installation requirement and precaution section below.
  3. Attach the carabiner from L-RG-50 to the O-ring, or D-ring of THE RF-1034 or RF-1036 roof anchor.
  4. Be sure the hook gate is not cross loaded, and the gate must close fully and securely when release.
  5. The L-RG-50 Rope Grab must be able to move freely on the roof anchor without hindering its movement.
  6. Attach the snap hook end of the shock absorbing lanyard to the dorsal D-ring of a full body harness.
  7. Make sure the snap hook closes properly and securely.
  8. Always check and make sure all connections and components are fully locked. Anchor strength requirement when using product for FALL

RESTRAINT applications:

Roof anchors installed for restraint applications must be attached to a roof member capable of sustaining a static load of at least 5,000 lbs. applied in any direction permitted by the restraint system when in use. Each roof anchor installation must independently be capable of sustaining this load.

ROOF STRUCTURE:

The anchor RF-1034 is designed and only to be used on installations for wood framed type roof structures capable of meeting the anchorage strength requirements as set forth by ANSI A10.14 and ANSI Z359.1. Consult with MSP before using RF-1036 on any other roof materials or structures.

CAPACITY:

The anchor is designed for use by single persons with a total weight of 310 lbs this includes(person+ tools), Only one fall protective system may be connected to the roof anchor RF-1034 at any given time. The MSP Synthetic Rope grab system model #L-RG-50 is rated for one person only.

PERSONAL FALL ARREST SYSTEM (PFAS):

With the use of this roof anchor RF-1034 be sure that all the components of this PFAS are met prior to using this product. Check other components of the Roofer’s Kit in a Bucket are compatible and safe to use with Roof anchor RF-1034. Please check system requirement section for more information

FREE FALL:

With the use of this roof anchor RF-1034, be sure that the roof anchor is rigged as part of the PFAS that user must limit free fall to a maximum of 6 feet in accordance to guide lines set forth by ANSI Z359.1

FALL CLEARANCE:

Enough clearance must exist in user’s fall path to prevent striking an object. The amount of clearance needed is dependent upon the type of connecting subsystem used (energy absorbing lanyard, self retracting lifeline, etc.), and the anchorage location. Please check connection of subsystem or component or equipment for detail.

CORROSION:

This product must be frequently inspected in adverse environments such as near sea water or other corrosive environments, acids,  chemical solutions, alkali and all other chemicals. Moisture richenvironment or elevated temperature may require more frequent inspections or replacement to ensure the integrity of this product. Corrosion damage effects the performance of the product, if the product is rusted, or corroded, the product must be remove from service immediately. Always have a competent person to inspect the product before, during and after each use. Contact MSP for any questions prior to installation of this product in chemical hazardous environment.

FIRE / ELECTRICAL HAZARDS:

Do not install roof anchors where the equipment or the user may come into contact with electrical power lines and/or a fire hazard environment.

TRAINING

Please make reference to ANSI Z359.11, Z359.15, Z359.18 for all applicable local, state, and federal (OSHA) requirements governing this equipment for more information on anchorage connectors, and associated system components. The user of this equipment must receive proper training on the correct application and usage of the product prior to installation

OPERATION AND USAGE

WARNING:

Do not alter or misuse this equipment in any way. Alteration and misuse can cause serious injury or death. Alteration and misuse voids warranty. Please contact MSP prior to using this equipment in combination with other components or subsystems not described on the above manual. Some subsystems and component combinations may interfere with the proper operation of this equipment. Avoid using this equipment around sharp edges, fire hazards, moisture rich environments, electrical hazards, chemical hazards and moving machinery.

WARNING: Always check with your physician to certify any reasons that will hinder your ability and fitness to safely absorb the shock from a fall arrest. Age and fitness seriously affect a workers ability to withstand falls. Pregnant women or minors are not permitted to use MSP fall protection products.

  • BEFORE Installation Prior to using this equipment, user must inspect for missing or damaged parts, see Figure 1. The steel side plates should be flat and free of corrosion. Check for installation of lag screws (6) or nails (12). Check for damage to the plates, D-ring or O-Ring. Do not use this product if inspection fails to be safe for usage. Destroy the product immediately to prevent usage.

REMARK:

The RF-1034 is designed as a single use anchor. It is intended to be installed, used, and destroyed to prevent future use. Not designed as a permanent anchor. The RF-1036 is designed as a reusable anchor. It is intended to be installed, used, and removed for future use.

PLAN

Be sure to plan your fall arrest or restraint system before starting work. Be sure to consult with a competent person for assessments of work application and proper equipment usage. Be sure to review the following list every time prior to planning your system usage.

SAFE AND PROPER ANCHORAGE:

Anchorage point selected must be rigid & capable of supporting the required loads.

FREE FALL LIMIT:

Personal fall arrest systems must be rigged to limit free fall to a maximum of 6 feet (OSHA and ANSI Z359.1) Avoid working above your anchorage level. A restraint system must be rigged such that no vertical free fall is possible.

FALL CLEARANCE:

It is required by OSHA that there must be sufficient clearance in the fall area to arrest the fall prior to striking the ground or other object. The clearance required is dependent upon the type of fall arrester connecting subsystem used (Shock absorbing lanyard, self retracting lifeline, horizontal lifeline system, etc.). Please refer to manufacturer’s instructions for fall clearance information.

PERSONAL FALL ARREST SYSTEM REQUIREMENTS:

This roof anchor must meet applicable OSHA, state, federal and ANSI requirements. PFAS’s when used with a full body harness must be capable of arresting the user with a maximum arresting force of less than 1,800 lbs. The maximum distance for extension upon the deployment of the shock pack (Deceleration device) is 42 inches (1.1m). Please reference to ANSI and all applicable OSHA requirements.

RESTRAINT SYSTEMS:

Restraint systems must meet applicable state and federal requirements. Reference ANSI A10.14.

AFTER EVENT OF A FALL:

All equipment subjected to the fall arresting forces must be marked (“Unusable”) and removed immediately from service. Equipment must be destroyed to prevent future use.

SWING FALLS:

The force of striking an object while swinging can be great and may cause serious injury or even death. Be sure to work directly below the anchorage to minimize Swing. Swing falls occur when anchor location is not directly above the user. Be sure to consult with a competent person or contact OSHA for better assessment of work environment and application.

SHARP EDGES:

Avoid working on sharp edges. The connecting subsystem such as harness, shock absorbing lanyard, self retracting lifeline etc. must be protected against abrasion from sharp edges. Do not wrap or loop lanyard around structural members. If working with this equipment near sharp edges can not be avoided, be sure to use heavy duty pads or abrasion resistant pads to prevent equipment from exposure to sharp edges. Always, inspect the equipment before, during and after each use.

PLAN FOR RESCUE & EVACUATION:

Prior to installation or operation, the user and employer must have a rescue plan and trained competent personnel on staff with qualified equipment to implement the rescue plan.

INSTALLATION REQUIREMENTS

Roof Anchor location selection plan:

Prior to installation of the roof anchor, a plan should be established as to where the roof anchors will be installed during the construction process, user should identify and understand how the roof anchor should be used.

The following are guidelines on locating roof anchors installation points:

Roof anchor should be located at the roof peak, if possible set the anchor at least 6 feet from any exposed roof edge. On smaller roof surface areas be sure to locate the roof anchor as far from the roof edge as possible.

  • Do not use this anchor near high voltage power lines or other energized power source.
  • Do not install roof anchors on fascia boards, or any unsupported roof structure such as gable overhangs or eves.
  • Minimum of 8ft spacing between anchors along the roof peak for anchor installation.
  • Do not use this Roof anchor if excess wear or structural deterioration is present.
  • Multiple roof anchors should be installed along gable ends for low pitched types roofs to reduce swing fall hazards. (Roof anchor must be installed at least 6 feet from the edge.)

ROOF FRAMING:

Roof anchor should only be attached to Roof framing members that are in good and solid condition. All members must be free of splits, cracks, large knots or other defects that may weaken the member. Do not attach the roof anchor to deteriorated or rotted unsupportive wood.

WARNING:

Previously nailed or screwed position on a rafter or truss should not be installed with Roof anchors. Be sure that the position of the new nail will not be used on any existing holes.

ROOF ANCHOR INSTALLATION:

On site work and State and Federal OSHA regulation must be followed in regards to roof anchor installation.

ROOF ANCHOR ATTACHMENT:

When making an attachment either on a roof peak or flat surface, adjust the side plates to match the surface that the roofer kit will be mounted on, position the anchor on the roof so the (Figure 3 & 5) screw holes along the center of the side plates are over a roof (framing) member (2×4 minimum). See (Figure 5). Then, push down to minimize any gap between the anchor and the sheathing. Each Roof Anchor must be installed using 10 16d x3-1/2” wood screws in center holes into truss. 20 16d wood nails are to be used for side holes into sheathing. Be sure that all the holes on the anchor plate are nailed or screwed prior to anchoring. See (Figure 6).
WARNING: Nails and lag screws must screw or hammer through the sheathing and into the roof member. If they do not, the anchor willnot hold the rated loads (5,000 lbs) and serious injury or death could occur See (Figure 6).

IMPORTANT:

If the RF-1034 is installed over old shingles, make sure it is nailed or screwed into the roof member (rafter or truss).

REMOVAL OF ROOF ANCHOR:

To remove the Roof anchor prior to shingling the area with the anchor. First, unscrew the wood screws Second, pull the nails and remove. The RF-1034 is a single use anchor, the anchor should be destroyed and removed from service after it has been removed from roof. The RF-1036 is a re-usable roof anchor and is designed to be reinstalled following above installation requirement and instructions.

MAKING CONNECTIONS:

When using a hook to make a connection, be certain accidental disengagement (roll-out) cannot occur. Roll-out occurs when interference between a hook and the mating connector causes the hook’s gate or keeper to accidentally open and release. Self locking snap hooks or self locking and self closing gate carabiner must be used to reduce the possibility of roll-out when making connections. Do not use hooks or connectors that will not completely close over the object. Do not use non-locking hooks or carabiners. Always follow the manufacturer’s instructions supplied with each system component.

  • The roof anchor should be located at the roof peak (when possible) and at least 6 feet from any exposed roof edge. On very small roof areas, locate the roof anchor as far from the roof edge as possible.
  • Do not install roof anchors on unsupported roof structures, such as eaves or gable overhangs. Do not install roof anchors on facia boards.
  • Roof anchors should be installed at 8 foot spacings along the roof ridge. Hip roofs require a roof anchor on each hip face.
  • On long low pitched roofs, multiple roof anchors should be installed along the gable ends (6 feet from the edge) to reduce swing fall hazards.
  • ROOF FRAMING: Roof framing members to which the roof anchors are attached must be in good condition. Members must be free of splits, cracks, large knots, or defects that may weaken the member. Do not attach the roof anchor to rotted or deteriorated wood.

RF-1034

  • Install anchors using (8) 8d x 3-1/2’’ nails. Must use all holes that are provided.
  • This product should be inspected after every use. For proper installation, fasten all holes in to truss with fasteners provided or alternate fasteners approved by manufacture.

RF-1036

  • Install anchors using (8) 3 1/2″ wood screws in the center holes and (20) 8d penny nails on the outer holes. Use all holes that are provided.
  • This product should be inspected aft er every use. For proper installation, fasten all holes in to truss with fasteners provided or alternate fasteners approved by manufacture.
  • ROOF ANCHOR INSTALLATION: Roof anchors must be installed in accordance with the previously discussed site plan. Site work rules must be followed regarding when an installed roof anchor is ready for use (i.e. after sheathing is in place).
  • ATTACHING THE ROOF ANCHOR: Spread the anchor base legs apart to match the surface it will be mounted on, either a roof peak or a flat surface. Position the anchor on the roof such that the 8 nail holes along the center of the legs are over a framing member. Then, push down to minimize any gap between the anchor and the sheathing and nail.

CONNECTING TO ROOF ANCHOR

When making connection to the installed roof anchor, be sure to only use a self locking snap hook or self locking and self closing carabiner.

  1. Always allow a minimum clearance level of at least 15 feet. Make sure no sharp objects are below the worker.
  2. Do not tie a knot when making connection of a lifeline to the Roof anchor.
  3. Do not pass lanyard or lifeline through roof anchor ring and hook back into lanyard or lifeline.
  4. During connection, make sure all snap hooks or carabiners are securely closed and 100 % locked. Only use a double locking snap hook or carabiner.
  5. When using an energy absorbing lanyard, always make sure that the shock pack is close to the person.
  6. When using a self retracting lifeline, make sure device is properly positioned so that retraction is not hindered, and be sure to connect SRL directly above the user to minimize swing fall hazards.
  7. Protect lifeline / lanyard from abrasion by sharp or abrasive surfaces.
  8. All connections must be compatible in size, shape and strength. Never connect more than one personal protective system to any single roof anchor at a time.

WARNING: Read and follow manufacturer’s instructions for associated equipment (ie. full body harness, shock absorbing lanyard, self retracting lifeline, etc.) used in your personal fall arrest system.

WARNING!!!

  • USER TRAINING – To meet standards, the user of this product must receive training from the manufacturer, or an ANSI or OSHA Certified Safety Instructor prior to using this product. All users of this equipment must be aware of the consequences of improper installation or use of this equipment. This user manual must be followed, however, it is not a substitute for a comprehensive fall protection training program. Training must be provided on a periodic basis and to be provided by a qualified and competent fall protection specialist or instructor.
  • WARNING: Ask your doctor if you’re fit to safely perform the job and also if your able to absorb the shock from a fall arrest. Only use this equipment if you are approved by your doctor with a written approval. Age and fitness seriously affect a worker’s ability to withstand falls. Pregnant women or minors are not permitted to use this equipment. User is responsible for safe work practice and proper use of this equipment.
  • IMPORTANT: Training should be repeated on a periodic basis. User should be recertified annually to ensure the latest updates and requirements by OSHA.

INSPECTION

Please READ IMPORTANT: If this equipment has been subjected to forces resulting from the arrest of a fall, it must be immediately removed from service and destroyed.

INSPECTION STEPS BEFORE USE:

  1. Check Roof Anchor for physical damage. Look carefully for any signs of cracks, dents or deformities in the metal. Check for bending, the roof anchor side plates should be flat. Inspect Rope grab, shock pack and webbing lanyard and O-Ring for damage.
  2. Check and make sure all connecting snap hook and carabiner are operating properly.
  3. Ensure the condition of the roof will support the Roof Anchor loads, anchor connected to rotted or deteriorated wood should not be used. (Supporting structure must withstand 5,000 lbs of tensile force)
  4. Inspect the Roof Anchor for signs of excessive corrosion, rusts or deformation.
  5. Inspect each system component or subsystem based on manufacturer’s instructions.
  6. Record the inspection date and results in the inspection log.
  7. Be sure to inspect for full retraction and release of the rope grab, if the rope grab mechanism is not working properly in securing the rope, do not use the product, label “Unusable” and destroy or remove from service immediately.
  8. Be sure to check rope for frays, burns or cuts, if found do not use the equipment.
  9. Only use a double locking snap hook and carabiners for anchoring.
  10. Be sure to use compatible harness, lanyard, or Self retractable lifelines when using these rope grab and roof anchors.

NOTE: DO NOT ALTER OR MODIFY equipment in any way, alteration and modification voids all warranty and liabilities on the product.

MAINTENANCE – SERVICING – STORAGE:

  1. Maintenance and servicing procedures (i.e. replacement parts) must be completed by a factory authorized service center. Authorization must be in writing.
  2. Unused roof anchors must be stored in a clean dry location.

(Tuff-Belt Anchor Strap is herein referred to as the “Product”) Each user must read instructions and warnings provided with the product at the time of delivery and prior to each use. Attention: Please consult your safety/hazard consultant or director! Read Instructions carefully for anchoring applications is extremely important. Using the product improperly can result injury or death. Anyone using the product is personally responsible for proper usage and safety and personally assumes all risk and accepts full and complete responsibility for any accidents, injuries, or deaths. If a fall occurs, the product must be disposed of according to manufacturer’s instructions.
  • GENERAL- This product cannot be used unless a qualified person has inspected workplace and determined that identified hazards can be neither eliminated nor exposed to them as prevented. Manufacturer of this product must include separate instructions. Copies should be accessible. Read and fully comprehend all instructions, labels and warnings for this product and other products intended for use with it. Failure to do so may result in serious injury or death.
  • USER TRAINING – To meet standards, the user of this product must receive training from the manufacturer, or an ANSI or OSHA Certified Safety Instructor prior in using this product.
  • PRODUCT SELECTIVITY AND USAGE – This product performs as stated when tested to the reference standards. It is part of a fall arrest system. CSA/OSHA ANSI standard Z359.1 certification is applicable to the device only. CSA/OSHA/ANSI has not investigated the anchorage system. Only a qualified person must select the components, materials and anchorage to match the system application, the work, workplace hazards and the environment.
  • Do not connect or combine with other components in ways which may hinder or defeat function of system. Please adhere to all instructions listed.

PRODUCT SELECTIVITY AND USAGE – Exposure to chemicals and corrosives will lead to weakened parts. If you are in doubt of product, you must consult manufacturer immediately.

  • Do not expose to sharp edges, abrasive surfaces, sparks, flame and heat above 185 degrees F. (85 C)
  • Keep safe distance from moving machinery and electrical sources. Metal parts conduct electricity.
  • Do not reuse product if subjected to fall arrest forces. Remove from use, label “UNUSABLE” until destroyed.
  • Do not misuse, abuse, alter or attempt repairs to this product. Disregard voids warranty. Only the manufacturer or persons authorized in writing by the manufacturer are permitted to make repairs or alterations. Manufacturer is not responsible for any mishaps or accidents if user makes alterations without consultation.
  • Have rescue and evacuation plans and means at hand to implement.

DIRECTIONS ON HOW TO USE – The MADACO Tuff-Belt anchor straps to be used with the following precautions.

  1. Make sure prior to wrapping the anchor object to have a competent person assess and evaluate the anchoring object as being capable of supporting a static load of 5,000 lbs.
  2. Wrap the anchoring object by inserting the small D-ring into the Large D-ring. (If same size D-rings are present on both ends, always connect the snap hooks to both D-rings. After wrapping the object be sure to wrap the anchor as short as possible to minimize the fall distance for easy rescuing if a fall occurs.
  3. Be sure when wrapping the anchoring object, the larger friction resistant pad is facing the object.
  4. Connect snap hook at shock absorber end to harness back (dorsal) D-ring and the other end of snap hook to be latched on to the I-Beam wrap.

Warning!!!

  • Anchorage must be at least 5,000 LBF (22.2 kN) strength per persons attached to it.

  • Space below user must be void of objects in potential fall path. Initial product length will increase by 3.5ft. (1.1m) when shock absorber fully extends during fall arrest. See separate instructions for description of fall path and fall distance.
    Always keep anchor directly overhead to reduce swing fall hazard.

  • Keep I-Beam wrap and lanyard as short as possible to minimize fall distance.

  • ALWAYS CHECK CONNECTIONS. This product’s D-ring meets ANSI Z359.1. They must be compatible with mating/matching connectors to reduce rollout possibility and must be securely closed and locked when coupled to another connector.

  • DO NOT make multiple connections to a single D-Ring.

  • DO NOT kink, twist or knot lanyard.

CARE AND STORAGE – Clean with mild soap and water. Air to dry. Do not apply heat.

  • Store in cool, dry and clean place away from direct sunlight and chemical vapors.
  • If inappropriately labeled, then remove from usage immediately. Label “UNUSABLE” until proper maintenance is performed or until destroyed.

WHAT TO INSPECT – Inspect for malfunction, torn or open cover, and missing, broken, distorted, damaged, corroded, weakened and worn parts. Imperative to seek for separate instructions for inspection details. If product is defective or if 6 months pass without formal inspection, remove from use. Label “UNUSABLE” until time for re-inspection, repair or destroy.

  • User must inspect or have product inspected before each use.
  • Separate competent person must formally inspect at least every 6-9 months.
  • Imperative to fill out each time on chart whether product passed inspection check point. (Please see grid chart)

MADACO “TUFF BELT” Full Body Harness Precaution and Instruction Guide Tool

WARNING! PLEASE READ CAREFULLY!!!

If you intend to use this product, you must carefully read and understand the following guidelines and stipulations. These instructions are never to be removed or altered without the 100% consent and authorization of MADACO Safety Products, Inc. You employer is required by OSHA to make sure that you, as the worker using this product, must receive proper training on how to use this product. If your employer cannot answer certain questions about the product, please contact MADACO, to answer any questions.

GENERAL PRECAUTIONS THAT YOU MUST KNOW

  • OSHA requires all workers to utilize fall protection when working a of six(6) feet or more.
  • OSHA mandates that any impact force in a fall, with the wearing of a legitimate fall arrest harness, cannot exceed 1,800lbs.
  • If you are not a properly trained and competent professional within the confines of safety, you cannot wear this equipment.
  • Employees cannot wear this product without the full consent and approval from his/her employer.
  • Equipment must be inspected daily. If there is something wrong with the equipment on that particular day, then you must not use this equipment until inspection has been cleared.
  • All Fall Arrest anchorage must be able to support or suspend a worker at 5,000 lbs(22.2kN).
  • You cannot alter or abuse this unit of equipment without the written consent of MADACO.
  • For fall arrest means, you must always keep the anchor point above and to the back of the D-Ring.
  • Fall arrest equipment must only be used for the purpose for which it was designed and intended for that particular situation.

General Requirements

  1. All MADACO “TUFF BELT” Full Body Harnesses are required to have warning labels and instructions visibly provided to you. If for some reason they are not visible, then do not use the product until you contact MADACO. Warnings and Labels have to be read before using the product.
  2. You must reference to ANSI standard Z359.1-1992, which applies to the standards of manufacturing. Also, please make reference to OSHA regulations on occupational safety for workers.
  3. You cannot use this product if you are not a trained professional on fall-protection safety.
  4. This product must also be visually inspected not only from you, but also from another trained individual prior to use.
  5. If the product shows any cut, wear, or structural change or tear, it must be replaced immediately! Do not try to replace or repair worn out parts with other accessories.
  6. ANSI regulations states that a worker cannot be more than 310 lbs in weight. If you are 310 lbs or more, you cannot use this product. (This includes the Body+ clothing + tools). If the user of this product decides to ignore this regulations, please sign a liability release form with MADACO prior to using this product at your own personal risk & liability.
  7. This product is designed and used for fall protection only. If you use this product for anything other than fall protection (ie. Towing, hoisting, leveraging) then you are at your own risk of injury or death.
  8. Do not use this product near high voltage environment.
  9. Do not use this product if you are a minor, pregnant, or have pre-existing health condition or problem.
  10. Have a rescue plan ready for back-up when the product is in use.
  11. Don’t knot this product with any component on this system. Knotting reduces component strength by 50 percent.
  12. Do not use this product near moving machinery, to reduce the hazard of entanglement.
  13. The material on this product is strong, yet it is highly recommended that there is protection from slag, hot sparks, open flames, and harmful chemicals.
  14. Make sure that do not Disconnect or alter the attached lanyard in anyway. If the user disregards the warnings for this product, it could lead to serious injury or even death. MADACO will not be responsible for any injury or death from improper use or neglect from the user.

SYSTEM COORDINANCE

MADACO “TUFF BELT” Full Body Harnesses are made to be used in conjunction with other MADACO “TUFF BELT” products. (ie. Lanyards, Rope Grabs, Rebar Hooks). Using other brand name products does not guarantee 100% compatibility and safe function.

Fall Protection Plan:

  1. Anchor point must be able to withstand a static load of 5000lbs (23kn) or greater. Anchor must be used in a location directly above the working position of the user in order to minimize swing falls or pendulum effect.
  2. 6 ft is the maximum free fall distance before activation. Falls with a greater distance than 6 feet will greatly increase the amount of force put on the body during the fall arrest.
  3. Make sure the fall path must be clear & free of obstruction and sharp objects. The maximum fall distance is 6 feet with additional 42” due to shock pack, including shock absorbing lanyard expansion.

THE IMPORTANCE OF PRODUCT INSPECTION AND CONFIRMATION:

MADACO TUFF BELT Full Body Harnesses are fully designed and capable to endure tough work environments and surroundings. Per industrial regulation frequent inspections must be done in order to see if there is any damage or malfunction on the harness or components. Check for corrosion or tears in the webbing, inspect for chemical damage, look for imperfections in the hardware, and make sure all components are in proper working condition. Log inspection results on inspection log located behind the warning label and user instructions on the harness.

WHAT WILL CAUSE WEAR & TEAR OF PRODUCT:

  • HEAT: Excessive heat will make the webbing become brittle and will weaken the tensile strength of the webbing.
  • CHEMICALS: The effect of chemicals on the product will produce change in color and potentially reduce the tensile strength of the webbing and hardware parts.
  • PAINT: If paint is allowed to penetrate into the webbing it may cause the webbing to become hard and brittle

FUNCTIONAL CATEGORIES DONE BY ACTIVITY

The use of MADACO TUFF BELT Full Body Harnesses can only be categorized in the following four (4) applications:

  1. FALL ARREST: According to OSHA regulations, a product of this nature must be used when working at a height of 6 feet or more. To minimize arresting force at the point of impact, it is recommended that a MADACO TUFF SHOX Lanyard is utilized.
  2. POSITIONING: This is the activity where the user is held in place while given the ability to work hands free by positioning his/herself directly in front of the task at hand. To maximize safety and ease of use of during this application the user should use rebar hooks and a self retractable lanyard for fall arrest.
  3. SUSPENSION: This activity is designed to lower and support the user while enabling hands free use of the harness. If suspension activity is occurring over a prolonged amount of time then use of a chair like feature or other additional support is recommended to decrease fatigue during use.
  4. RETRIEVAL: When in tight and confined spaces of work where workers are being lowered into manholes, caves, etc. Certain MADACO harnesses can be used along with retractable rescue lifelines and a tripod

IMPORTANCE OF CLEANING AND MAINTAINING

The better you take care of the product, the longer it will last. Keep product in proper storage, meaning out of high temperatures, steam-filled rooms and extended exposure to sunlight.

  • A) If oil or paint gets on the webbing, clean immediately with mild water and soap.
  • B) Do not place or hang up in areas exposed to fumes or dangerous elements.

Warranty:

Equipment offered by MADACO is warranted against factory defect in workmanship and material for a period of 1 year from the date of purchase. This warranty does not cover damages on the equipment as a result of abuse, damage in transit, or damages beyond the control of MADACO Safety Products Inc. Proof of purchase is required for all warranty actions. Madaco reserves the right to inspect the product submitted for misuse prior to sending out replacement.

USER’S RESPONSIBILITY

Missuse of Madaco safety products can result in serious injury or death, for that reason Madaco safety products Inc. will not be responsible for any liability or claims on any product. All users that are not fully trained and informed on the correct application of Madaco products assumes 100% personal liability and relieve Madaco, Madaco employees and any affiliates of all liability. This means even in the case of serious injury or death MADACO Safety Products Inc. will not assume any liability for your inability to follow directions and accept professional training from the outset.

To use harness properly wear close fitting as shown

Be sure that all straps and connection points are untangled and tighten to the users body. Do not wear harness in a baggy loose fashion it will not be effective during a fall if harness is not used as it is directed

User Instruction and warning for Tuff-Shox, Tuff- Lanyard system

YOUR WORKING AREA MUST BE SAFE AT ALL TIMES! PLEAS READ AND FOLLOW ALL INSTRUCTIONS AND PRECAUTIONS. FAILURE TO DO SO WILL RESULT IN SERIOUS INJURY OR EVEN DEATH.

  • A Personal Fall Arrest System(PFAS): A system that is used to stop a worker from falling at any work level. It consists of an anchorage device, connector(s), body harness, shock absorbing lanyard, lifeline(s), or a suitable combination of the above.
  • Anchorage: A secure or confounded point of attachment for the personal fall arrest system.
  • Body Harness: A system of straps that is secured about a worker. The harness is designed to distribute the fall arrest force over at least the thighs, pelvis, waist, chest, shoulders.
  • Connector: A device used to attach the components of the PFAS. .
  • Shock Absorber: A part of the lanyard designed to lessen the impact during a fall arrest situation.
  • Lanyard: A Flexible line of rope, wire rope, or strap used to secure the harness to the anchor.
  • Lifeline: Is a flexible line for the purpose of connection to an anchor at one end to hang vertically, or at both ends to secure strong horizontally.

CHECK BEFORE YOU USE

  • Visually check all snap hook connection(s) and connector(s)
  • Make sure that snap hook keepers are free of risk burrs, clean, and not distorted or bent out of shape.
  • Make sure snap hooks are not cracked.
  • If the entire unit in general seems to have abrasions and looks like it was worn excessively, then do not use this product.

WHAT NOT TO DO WITH LANYARDS!!!

  • Do not attach a lanyard onto itself or attach multiple lanyards together.
  • Do not wrap a rope lanyard around a beam or sharp structural member.
  • Do not put a lanyard in close contact with high temperatures.
  • Do not use a lanyard in applications that deal with towing or hoisting.
  • Do not attach multiple snaps onto a D-Ring.
  • If possible, do not tie multiple knots into a lanyard.

WHAT NOT TO DO WITH LIFELINE(s):

  • Do not tie a lifeline back to itself.
  • Do not use a lifeline around a beam or other sharp objects.
  • Do not use a lifeline in close contact with high temperatures.
  • Do not use a lifeline without always attaching the snap hook to a proper anchor point for intended use.
  • Do not use a lifeline in applications that deal with towing or hoisting.
  • Do not tie multiple knots onto a lifeline.

GENERAL PRECAUTIONS YOU MUST KNOW ABOUT HARNESS(s):

  • Always make sure to visually check that all appropriate buckles are properly closed up before usage.
  • Fall arrest locking hooks must be used.
  • D-Ring must be centered in the back of the fall arrest harness.
  • Do not attach multiple snap hooks to a D-Ring.

CLEANING AND MAINTENANCE GUIDELINES:

Wash nylon webbing with warm water and a mild detergent. Do not use harsh chemicals. Let nylon webbing to “air” dry. Adding heat should never be done!!! Make sure to timely lubricate the connectors and snap hooks with lubricating oil. When storing, make sure to keep it away from sunlight and high temperatures.
